Mark returns to work, physically and emotionally scarred. Carla goes into labor again, about eight weeks prematurely, and delivers a baby boy. Doug thwarts Carol's efforts to celebrate her birthday quietly. Carter's doubts about whether his future lies in surgery grow. Maggie tries to interest Mark in various self-defense devices, resulting in Carter accidentally pepper-spraying himself.

Two main storylines are Carla's labor and Greene's return.
Greene comes back after a week and he is under post-dramatic stress as he is not feeling himself comfortable. Doyle starts to help him by bringing different things for self-protections but he says no (and Carter, passing by, manages to push on the pepper-gas and aim it to his eyes) Weaver tries to get better security and make the ER more secure but Greene feels that does not help - the problem is the doctors and patience who keep them in too long, who do not work as quickly as the should.
Carla's labor starts, two month too early and the babyboy's life is on danger as he has many health problems. Carla is coping quite well, she is strong woman, but they two will have hard times ahead.
Carter is more and more having doubts is he on the right place as he feels that he wants to care about the patience, do like they want not so much go and cut everyone. And his relationship Anspaugh is worse. And he really thinks that ER might be the place he wants to be.
And the episode ends with Carol's surprise party.
Great episode. Dr. Greene returns after being attacked in the mens restroom.

Dr. Greene return to work after being attacked. He now thinks that if the Dr.'s move people in and out faster that this will reduce other people's risks of being attacked by an upset patient like he was.
He later finds out that none of his patients or their families were responsible for his attack.
Carla goes into labor, and gives birth to her and Peter's son.
The baby is premature and is suffering complications.
Doug tricks Carol into believing that he is saving her from a surprise Birthday party with her co workers and relatives.
He says he will take her out to dinner, they go to her house so she can changed and everyone is there.
Dr. Carter considers leaving surgery for emergency medicine.
Great episode.
I love the slow mo shot when Carol goes to make a wish and blow out her candles. The camera looks up at Doug then back to her, as if to say Doug is what she is wishing for.
I just love it.
