Looks like Holmes has made a breakthrough. As Afredo and Joan both pointed out although Homes says that he was not responsible, his actions (problems concentrating on the task of breaking into the car, belie his true feelings that he actually does feel responsible for what happened to Bell. Alfredo was also perceptive enough to realize that it would do some good for Holmes to practice empathy by becoming a sponsor. The disagreement with Joan about the unforeseen consequences of leading with the head and not the heart also hit home for Holmes. He saw how Joan was able to honor her confidentiality agreement with her former client and still find a way to expose the killer and minimize the impact on her former clients' life. The old Holmes would have gone ahead and used the information anyway and not care about any collateral damage that might occur.

      "Randy.
      Name or adjective?
      What?
Short for Randall or state of sexual arousal?

      Made me snigger...

What does Randy say right at the end of the episode? I missed it. It's during the discussion about Sherlock becoming Randy's sponsor.

      Holmes:
      Are these terms acceptable?
      Randy:
      You're sober.
      You have been
      for a long time now.
      Of course
      I find the terms acceptable.

Actually he was the uncle--Dan Aykroyd was the father. Close, though. :)
