When Reese realizes the Army is too much for him to handle, he deserts and dresses as an Afghani woman. But Lois is on a march to find him, and travels to Kabul to hunt him down. Meanwhile, Malcolm volunteers at a veterans hospital as a way to make up for his guilt of making Reese leave.

Reese's hair has grown back after he got it shaved off... so it's either been a long time since he left, or the show is just ignoring that fact.

There is no "Previously on..." however Hal does a very quick summary with some flashbacks during the cold open.

First episode of "Malcolm" to be presented in HDTV. Previous episodes were in Fox High Resolution Widescreen.

First season premiere since the pilot where the episode is not mainly off-site (away from the home), as only Reese and Lois are away from home.

For this season, Christopher Kennedy Masterson (Francis) remains credited, but only appears in a few episodes, which always involves the family instead of his alternate locations (Marlin Academy in seasons 1-2, Mamu in season 3, and the Grotto in seasons 4-5)

Hal: You've got to accept that some people can do things you can't. Heart surgeons, rocket scientists, that lady at the mall that carves your name in to a grain of rice.
Lois: How does she do that?
Hal: No one knows. (edit) (Malcolm and Dewey show their lists of "Mean Things Reese Did" to each other)
Malcolm: "Lighter Fluid Donut"--That's worse than stealing a girlfriend, right?
Dewey: I don't know. Were you in love for the first time with that donut?
Malcolm: Shut up, Dewey! I had every right. In fact, I owe him!
Dewey: So you're saying if he dies, then you'll be even?
Malcolm: I didn't say he was going to die!
Dewey: Malcolm, he almost killed himself with Bisquick once.
Malcolm: I'm just saying he deserves whatever he gets. (Sighing) No, he doesn't. That's something that Reese would say. I'm not Reese. Just because he's horrible doesn't mean I have to be. I'm better than that.
Dewey: Wow, for a brother-killer, you sure are conceited. (edit)

At the end of the episode, when Reese, Malcolm, and Dewey are on a rooftop launching dirty diapers, it recalls their slingshot stunt from season 2 episode "Halloween Approximately."

When Reese is looking at the "map" on the bubble gum wrapper, he is in fact, looking in the correct position on the map.

When Malcolm tried to play ping-pong with the old veteran and the ball goes into the man's mouth (and we hear the man swallow it), we see the same man in the fight scene, so we know he did not choke and die.
