Citizen Toxie: The Toxic Avenger IV follows the Toxic Avenger (David Mattey) on an comic adventure to save Tramaville. After 10 years of peace in the town, the Diaper Mafia has taken the Tromaville School for the Very Special hostage. Before the hero can rescue the students, an explosion opens a portal to another dimension and sends Toxic Avenger to Amortville. Taking his place in Tramaville is Toxic Avenger's evil mirror image, the Noxious Offender (David Mattey). Noxious Offender goes on a killing spree while Toxic Avenger is trapped. Sgt. Kabukiman (Paul Kyrmse) must lead a team of superheroes while Toxic Avenger looks to Sweety Honey (Lisa Terezakis), Tito the Retarded Rebel (Michael Budinger), and Chester (Joe Fleishaker) to help him get home. In Tramaville, Noxious Offender has a tryst with Toxic Avenger's wife, Sarah (Heidi Sjursen) and she becomes pregnant with two babies from two fathers, thanks to the help of of her gynecologist (Corey Feldman).
