As the film begins, Frank Leone is closing in on the end of his sentence at a low-security prison. However, before he can be released, he is kidnapped and put into the violent maximum security prison called Gateway Prison. Leone is there because the prison's warden, Drumgoole, has a grudge against Leone. So, he arranges for Leone's time at Gateway to be miserable. The warden confiscates letters for Leone, gives him dangerous roommates, and rewards other inmates for ruining Leone's life inside and outside of the jail. With the help of sympathetic guard Captain Meissner, Leone fights back against Drumgoole's awful treatment in a series of physical and mental battles.
