Passion Fish is a drama film that focuses on life's second chances. May-Alice Culhane (May McDonnell) is a soap opera star who is in a horrific car accident. She undergoes surgery and when she wakes up in the hospital, she is initially confused as to how she got there. Once reality sets in and she realizes she is paralyzed, May-Alice becomes ornery and angry. When she is released from the hospital, she has no other choice but to return to her family's abandoned home in Louisiana. Feeling sorry for herself, she begins drinking heavily, causing her caregivers to become frustrated and quit on her. May-Alice's life changes when a nurse named Chantelle (Alfre Woodard) takes over as her caregiver. Chantelle is a spunky nurse who does not stand for May-Alice's grumpy behavior. As time passes, the two begin to share secrets and form a bond that May-Alice has never experienced with anyone before.
