Deacon decides to set some boundaries in his relationship with Rayna, which makes her start to think the time has come to move on. Also, some parts of Deacon's past come back into the forefront of his mind, Juliette continues to struggle with her mother's addictions, and Scarlet is put in the middle of the growing tension between Gunner and Avery.

A woman from Teddy's past returns, which tests his and Rayna's marriage. Also, Rayna performs with Deacon at Teddy's fundraiser. Meanwhile, Juliette makes an appearance on Good Morning America, and Gunner makes a connection with his publisher's assistant.

Michael gets back in touch with an old friend, Gene, who had recently fallen in love while traveling in India. After Gene collapses, Michael takes him in for treatment while his mother and new girlfriend squabble as they both take care of the important man in their life. A new patient comes back with a link to one of Kate's old patients.

Michael continues to worry people at his hospital and upset people at Anna's clinic. After people show up at his personal clinic looking for free health care, Michael ends up at Anna's memorial service and starts meeting people in her life. Before he can leave, though, a young boy arrives with a hurt elbow, which turns out to be infected and the child's life goes downhill from there. Meanwhile at Michael's practice he has both good and bad news for a family friend and his wife, as she's pregnant but also has a brain tumor.

When Michael Holt runs across his ex-wife Anna Paul, he invites her to join him for dinner and the two share a nice evening. When Michael calls her clinic the next day, he is stunned to find out that she died two weeks prior. Anna begins to visit him from the afterlife regularly telling him that she died in a car accident and has left much work undone. She asks him to help continue running her clinic, which caters to low income patients.

Michael decides that he cannot handle Anna's visits and asks a find of his sister Christina's to exorcise her ghost from his life. But when the times comes to say goodbye, is he ready to let her go forever?
