Addison and Sam work to bring the practice out of bankruptcy, as all of the doctors look for additional clients and new income streams. Violet's friendship with Cooper is on the brink.
A young couple is having difficult conceiving a child. In the process of getting help with this, a discovery is made that the couple are half-sibs.

In this episode of Private Practice the following happens. Naomi is still not speaking to Sam or Addison unless she has too. Lisa & her Husband (Brian) come to the practice to find out why they haven't been able to get pregnant after about 5 years of trying. When they get back their genetic testing they discover that they are actually brother and sister, they just don't know it. Addison and Naomi learn that they were both conceived using a sperm donor and it was the same one. Sam has a patient named Kurk, who wants to die (he has a lung condition that will kill him). Lisa and her husband decide to stay together, although they can't have kids together. Lisa says that she wants her tubes tied. We later learn that Brian knew they were related before they got married. It breaks Lisa's heart and she collapses into the arms of Addison. Cooper asks for Violets help when he discovers that he has head lice. He admits to her that he is sleeping with Charlotte. Kurk has his tube removed and h then dies with his father by his side. At the end of the episode Pete sleeps with the new girl (who is doing Dell's job), and Naomi goes round to Sam's house and they also have sex (angry sex at that).
The conflict over doing what's best for the patient or the practice- great idea. I also loved how the couple turned out to be brother and sister. hide show
I loved how each doctor was dealing with doing what was right for the patient or the practice. They came up with some perfect examples of ways that helped the practice but not the patients, I especially loved the case about keeeping the guys son alive. The board with the dots was so funny. I loved how Sam realized he needed to have faith and do what was best for the patients. The brother and sister case was so good, I really wanted to know how someone would react to that type of news. I found that I felt really bad for them and sympathized with them. Cooper confiding Violet was great, and I was kind of suprised to find out he was in love with Charlotte.

This might probably be the best episode in this series so far. I would characterize this installment as one with intense scenes and powerful performances by the cast. It was a unique mix of well paced drama and a bit of comedy. The story kicks off with Sam taking charge of bailing out the practice by asking his team to aggressively sell themselves. Pete draws business by using a fat burning contraption, Cooper by charming potential patients at a health fair, and Sam by doing executive physicals. Addison and Naomi attend to seemingly perfect couple, who later realize they were conceived from the same sperm. The docs trouble add on when the female asks Addy to tie her tubes.
The prime premise of this episode was to convey how far the doctors were willing to overstep moral and ethical boundaries to add more cash. This is clearly reflected when Sam, against his own wishes complies to a father's decision to keep his terminal son alive. The final scene when Sam finally convinces the father to pull the plug is a total tearjerker.
Some other subplots such as, Cooper winning back his friendship with Violet, after an elaborate speech - confessing his love to Charlotte, although he knows that Charlotte doesn't feel the same way. Addy and Naomi's falling apart is touched upon this episode. I missed Dell this episode. Also, the character reversal of Pete is somewhat surprising. All in all, a perfect episode after an average season opener.

Whenever I try to turn someone onto Private Practice I say that if you can get past the first five minutes you will experience a good show. Once again that was the case tonight as we had a typical Grey's Anatomy girl-talk scene, before getting into what makes this show great, the unique medical cases the doctors have to deal with. St. Elsewhere, Chicago Hope, ER, these shows did not have the kind of cases Private Practice has produced in its limited run.
Tonight we saw Sam have to deal with a father keeping his son alive at all costs, even against his own volition because he could not let go. We also saw Addison and Naomi have a fundamental dispute over a married couple that was conceived from the same sperm. I liked both cases, even if we had to deal with the annoying Fran Kranz in the latter. We saw a different side of Addison here as well instead of her usual powerful, strong-minded self, we saw signs of weakness in both her conflict with Naomi and breaking down on the phone to Swat Officer Kevin.
Private Practice is sculpting out the foundation for a really strong series, let's just hope the ratings improve so a lengthy run is possible.
