The episode begins with Kaori explaining to Dr. Tomoe her mission in stealing the three pure heart crystals. He then creates a new monster to steal a pure heart crystal from their next target.


Serena then has a nightmare of her losing her transformation again, just as she did in the previous episode. She notices that the Silver Crystal's shine seems to be fading. She wakes up hysterical, with Luna assuring her that she shouldn't worry. Luna mentions that maybe speaking with Darien will help, so Serena sees him the next day.


In hopes of cheering Serena up, Darien brings her to an abandoned house where all the stray cats in the neighborhood live. There is where they meet the new pure heart target, a girl who feeds all of the abandoned cats. She tells Serena that the abandoned house will be torn down soon, so she promises to help find a new home for all of them with Darien.


Later Serena hopes to persuade the Scouts into adopting a cat. Each of them creates an excuse as to why they can't house any of the cats. Mina explains Artemis is already a handful and Raye says that the temple is a sanctuary to the birds so a cat wouldn't work. Lita oddly enough, mentions that her mother is allergic to cats. This is odd since in the Japanese version, Lita's parents died in a plane crash. Amy can't take care of a cat due to her studying and Serena makes up an excuse about Luna being too much to feed already. The scouts then head off to the abandoned area, to help find food for the cats.


The pure heart target walks home, with two stray cats following her inside. They knock over a puzzle of a cat, causing one of the pieces to fall. When the girl puts it back into place, the new evil monster Dr. Tomoe created comes to life. It's an evil cat woman by the name of Nekonei, set on stealing the girls' pure heart. The girl then runs away into the abandoned home area, where the Nekonei follows her. The scouts are outside and notice all of the cats acting strangely, running into the abandoned house where the monster is. They run inside and discover the heart snatcher, causing the battle to begin.


Serena tries to transform but is shocked to realize that she can't. The Scouts instead transform to help defeat Nekonei. Mars and Jupiter fire attacks but are unable to defeat Nekonei. Nekonei steals the pure heart crystal, put are attacked by Uranus and Neptune.

Sailor Uranus and Sailor Neptune examine the crystal and realize it isn't a pure heart one. They hand it back to the girl, and attempt to take off. Behind them, Nekonei begins to form back and Venus calls out to Uranus for their help. Uranus turns around and explains that Venus is misunderstood because her and Neptune aren't there to help the Scouts. She tells them that they're only after the pure heart crystals. Neptune tells the girls to leave them alone, and the two disappear.


Just as Nekonei is about to attack Sailor Moon, Tuxedo Mask appears. He attacks but Neko is unaffected and attacks him. With the odds against him, Tuxedo Mask tells Serena to run away. Serena refuses and the two hold hands, causing a quick vision of the Moon Kingdom to appear. Suddenly there is a burst of light between Serena and Darien, as the legendary Spiral Heart Moon Scepter appears between them. With the scepter and a new broche, Serena is able to transform again. She uses her scepter, and Nekonei is defeated.


In the end, Serena gets in touch with the animal center where they offer to take in all of the abandoned cats.
