Dr. Kelso organizes a coffee shop to be built next to the hospital, Coffee Bucks, and it quickly becomes a hangout for all of the staffers. One of the waiters is none other than ex-doctor, Cabbage, who quickly goes back to being friends with J.D. Dr. Cox however is disgusted with the coffee shop promoting the hospital, especially when it comes to tips and is inspired to get a tip jar himself as treating patients deserves profit more than pouring hot water on beans.
Kim is searching for a job in the area, since staying in a teaching hospital forever would be a waste of life. Carla meanwhile is recuperating in the hospital, and letting everyone hold Izzie. When Elliot asks, Turk doesn't let her because she drops everything, and as a test throws a ball at her which she subsequently misses. Carla says that she doesn't know if she'll ever return to work if it meant parting with her daughter.
Elliot is treating a private practice doctor and while she's calling the shots at the moment, Dr. Cox promises that he will turn on her and start treating himself as they all do. Elliot then confronts Dr. Turner, who promises that she can do everything, and she decides against surgery, Cox sill insisting he'll stab her in the back. When he opts for surgery behind her back, Elliot is furious and snaps at him, Dr. Cox gloating.
With the basketball court being torn down to make room for the Coffee Bucks, Turk and J.D. play across town and when they lose, Turk refuses to buy J.D. some cotton candy and when one of the opponents says he'll pay if Turk gives him a physical, Turk gladly accepts the cash. When J.D. confronts him about this, Turk doesn't seem to care and in order to buy the cotton candy, J.D. agrees to look at the vendor's rash, which he declares as benign. J.D. feels even guiltier when an ambulance wheels the vendor in and J.D. thinks he misdiagnosed and screwed up the guy's life. When it turns out he was shot, J.D. is still mad at Turk, who has changed his ethical ways, not buying him anything and giving medical advice away. When Turk refuses to pay back the money, J.D. tattles on Turk to Carla, and in his rage pegs the ball at him, which knocks Elliot over but she manages to catch the ball and hold Izzy.
Dr. Kelso refuses to give the Janitor and the cafeteria and sanitation staff a dental plan and when the Janitor finds out that Coffee Bucks has a dental plan, he gets a job there as the rest go on strike from their duties as well. Within a day, the Janitor becomes manager and refuses to serve Dr. Kelso, who he knows to be taking muffins, as well as not giving certain staff members dental plans. Eventually Dr. Kelso gives in and the Janitor resumes working at his remedial job.
When Elliot finds out that Turner and Cox were in on it together, Cox teaming up with the profession he hates in order to annoy her, Elliot is mad, but Turner respects her for not agreeing with him all the time and offers her a job in private practice. While Elliot has enough integrity not to sell out, she is swayed by the fact that she will no longer have Dr. Cox as a superior and takes the job.
Turk confronts J.D. about his tattling and points out that if Carla doesn't return to work, he'll have to support the family on his own and hence needs all the money he can get. J.D. says he'll be fine, but not without bringing up an old story about Turk sleeping with his college girlfriend. Turk tells J.D. nothing happened and says that J.D. too will be fine raising Kim and his kid. With this in mind, J.D. goes to help Kim find a job, but Kim was already offered one... in Tacoma, Washington.
