J.D. and Turk's friend from college comes to stay with them for a few days to go to their frat brothers' wedding. On his first night there, the guys get drunk and the next morining, Spence blurts out to Dr. Cox that Jack is actually his son.
Meanwhile, during rounds, Elliot accidently throws a needle in to Dr. Kelso's face, causing him to pickon her relentlessly. When she stands up to him, Dr. Kelso reveals that he is not only torturing her because she struck him with the needle but because Elliot is far behind and should consider quitting and changing professions.
The Janitor stops J.D. to tell him that he makes more than him. Meanwhile Dr. Cox is furious at Jordan and eventually breaks up with her. Jordan, however doesn't take this for an answer and continually follows him around the hospital until he takes her back. While fleeing from Jordan, Dr. Cox runs into Elliot, who opens up to him about Dr. Kelso getting to her and breaking her spirit. Distracted, Dr. Cox ignores her, causing Elliot to stress out a lot more.
On backup call, Turk and J.D. decide to get drunk with Spence in a hot tub, however after a freak accident, they are called in to work. Dr. Cox is furious at them aswell and kicks them out of the hospital. On their way out, J.D., in a bad temper, tells the Janitor that while he makes more, he is worthless and doesn't make a difference in the world.
When Turk and J.D. confront Spence, he turns it back on them, making them realise that they weren't forced to go out, but instead were sick of their jobs. Having to make up their lost shift, J.D. and Turk miss the wedding but now feeling guilty, J.D. apologises to the Janitor and is manipulated into eating off the floor.
When Jordan confronts Dr. Cox on their obviously dysfunctional relationship, Dr. Cox forgives her but doubts his performance as a father. In a better mood, Dr. Cox then forgives J.D. by calling him "not the worst resident in the world", although light on the evidence to back that up. He comes clean to J.D. about his doubts as a father and J.D. convinces him to test his parental skills as if Elliot was his daughter, with Dr. Kelso yelling at her. About to break down, Elliot is shocked when Dr. Cox walks up and punches Dr. Kelso in the face, knocking him to the floor, before going home to see his son.
