J.D. is bummed out by his life, but takes solice in the fact that Dan is coming and though he doesn't appreciate his brother's visits, he looks forward to his brother's MORE depressing life. However, he's startled to find out that Dan has matured, and has settled down with a proper job and a house. Though J.D. holds all of this in, when Dan buys him a car, so that he can visit Sam more often, J.D. tries to destroy the car and yells at Sam stating that Dan's supposed to be the screw-up not J.D. Dan calmly tells J.D. that he needs to "grow up".

J.D., unsettled by this comment, goes around asking people if this is true, and everyone tells him that he needs to mature. Turk explains that since he's a father now, he needs to stop acting like a child all the time, and take more responsibilities. J.D. reluctantly accepts this truth, and in his first act of maturity, invites Dan to go see his son with him.

Meanwhile, Elliot chides Dr. Cox for being a hypocrite, but the tables are turned, when she tells an anorexic patient that she needs to gain weight. Cox points out that Elliot TOO needs to gain weight. Elliot decides to end her hypocrism, and decides to eat more food, but doesn't want to, as she's proud of her sleek figure.

The Janitor meanwhile, becomes more environmentally-friendly after watching An Inconvenient Truth. He imposes protoccol and punishments for people who disobey environmental rules. However, in the end, he himself becomes more lax, and throws away recyclables instead of recycling them. Carla reassures Elliot and the Janitor, that they're just human, and its natural for them to act as such.
