After a hard day of work, Dr. Cox has the burden to tell his son Jack a good night story, so disguises his hard day as a fairy tale about the little village Sacred Heartlia with Carla and Turk as the two-headed witch Turla, Elliot as the princess and J.D. as the village idiot.

J.D. is able to avoid being suspended for working late because he has had no interaction time with the patients and therefore there are no potential liability problems. However, Doug is sent home without pay for working late, despite the fact that he works in the morgue and all of his patients are dead, making it very unlikely that he will be sued for anything he does. edit »
In the beginning of the episode when J.D. is talking to Elliot he says "a month ago you were going to marry him...and then it all fell apart when we had that almost moment in the oncall room." However, moments later when he is talking to Dr. Cox he says "A FEW MONTHS ago Elliot and I almost kissed...", clearly contradicting the time frame he had just himself verified. edit »

When this episode aired on NBC, it was labeled as the Scrubs finale, however, this was not the finale, as Zach Braff revealed in his Myspace blog that there are already 18 more episodes in production for a season. In contrast however, this is the last episode that aired on NBC, as season eight is going to air on ABC. edit »

Ted: (As the hunchback) I tell most people it's because I'm under an evil spell, but really it's a self-confidence thing.

Janitor: Eeee, thanks man. Do you know how dangerous this is? My uncle's whole family was killed by an untied lace. They were running down the stairs and they tripped and a mad man hacked them all to pieces, I owe you one.

This is an allusion to the character Inigo Montoya (played by Mandy Patinkin) from the 1987 movie The Princess Bride, where Inigo was trying to find the killer of his father.
