Throughout this episode, multiple characters said that Andy Diggle was (presumed) dead for eight years; however, in "The Return," a flashback scene set circa 2010 shows John and Andy working security for a party that Thea attended. Therefore, Andy could have been (presumed) dead for only 4-5 years.
When Oliver hangs upside down to enter the door code, Felicity tells him to punch in 4-5-8-7. But since Oliver was upside down, he actually hit 4-5-2-1.
At approximately 33 minutes into the show, during Thea's fight with Lonnie Machin, her mask gets twisted to the side, and then corrects itself during the jump-cut. Thea never touches it, and the timing is too fast for her to have done it off-screen.

On the ground shot, Sara's long hair is seen fluttering in the breeze caused by her fall, whereas in the overhead shot, her hair remains relatively still.

Diggle jumps into Oliver's car to escape Cyrus Gold and says "He picked me up like I was nothing." But Cyrus never picked Diggle up; he punched him and sent him flying across the room.
When Quentin comes to Laurel's aid after she was pulled over, the police officer tells him that she was .6 above the legal limit. Which would mean she blew a .68. This is not really possible, .4 is the lethal limit for more than 50% of adults. He should have said she was .06 over the limit.
