This episode starts with Commissioner Gordon in a dentist office for his annual check up, only to see that his dentist is really the Joker.
Gordon tries to escape, but then Harley ties him to the chair and gags him. Joker is about to drill a hole into Gordon's head when Batman arrives.
He then shows a set of fake teeth that Joker sent him and says that his clue was too easy. Harley then gasses Batman while Joker tosses a bomb into the room before they escape. Luckily, Batman tosses the bomb out the window before it explodes.
Back at the Joker's headquarters, Harley tries to flirt with Joker, who is too busy thinking up a new plan to get rid of Batman.
He comes across a plan called "The death of a thousand smiles," where he drops Batman into a tank of piranhas. But then he remembers that he scrapped it because piranhas can't smile. Harley claims that she knows how to make someone smile until Joker tosses her outside.
Harley starts to wonder when her life went wrong and she blames Batman, who was always getting in the way of her and the Joker.
We then cut back to when Harley was Harleen Quinzel, a young psychologist who had just started working at Arkham. On her first day, she receives a flower on her desk with a note from the Joker. When she asks him why he put it there and he explains how he likes her.
For the next few days, Harley talks to the Joker about his personal life. Joker talks about how his father use to beat him up all the time and the only time he saw him really happy was when he took him to the circus when he was seven. He goes on how his dad really liked the clowns when they dropped their pants, so the next night he goes to his dad with his pants down to try to make him laugh, but instead, his dad breaks his nose. But the Joker said that that was the downside of comedy, you're always getting hits from people who just don't get the joke.
Harley soon starts to believe that the Joker is just a misunderstood soul who was driven to a life of crime. When she saw Joker all beaten up after his latest bout with Batman, Harley knew that she had to help him. So she stole a bunch of stuff from a costume shop and became Harley Quinn and helped the Joker break out of Arkham.
In the present, Harley comes up with an idea to impress the Joker. She films a live televised message to Batman that she has given up her life of crime and she needs Batman's help to stop the Joker's latest scheme.
Batman meets Harley at the docks when all of a sudden the Joker arrives shooting at them. Batman fights off the Joker, only to see that he's just a robot and Harley knocks him out.
When Batman wakes up, he is hung upside down above a tank of piranhas. Harley explains how that this is one of the Joker's plans, only with him upside down, it will look like the fish are smiling.
Harley then goes on how she just wants to be with the Joker, which makes Batman laugh. He explains how the Joker is just using her, but Harley doesn't believe him. She talks about how the Joker told her secret things about his life. Batman explains that he only tells the stories to gain sympathy. He talks about one story the Joker told a parole officer. It's the same story he told Harley, only instead of the circus, the Joker said his dad took him to an ice show.
Harley still doesn't believe Batman and she calls up the Joker to tell him that she has Batman. When the Joker arrives, Harley expects him to be happy that she caught Batman, but instead, he is furious because he wanted to capture Batman by himself. The Joker is so angry that he shoves Harley out a window.
Joker then unties Batman and apologizes for Harley's actions. But just as Joker is about to leave, he sees this as an opportunity to kill Batman once and for all. He takes out a gun and is about to shoot him, until Batman rams him and the gun goes off and destroys the piranha tank.
The Joker escapes and Batman chases him all the way to the roof of a moving train. The two have a huge fight until Batman shoves the Joker right off the train and watches as he falls into a smoke stack.
Back at Arkham, Harley is sent to her cell all bandaged up after her fall. She goes on how she now knows that the Joker is just a evil criminal who was using her the entire time. That is until she sees a flower by her bed with a "get well" card attached to it and she knows that the Joker gave it to her and she lies on her bed with a smile on her face.
