Goof: During the discussion in the Commissioner's office with Sergeant Daniel Vega and Cadet Martin Brown, the Commissioner says Sergeant Gormley will get busy on Sergeant Vega's reassignment. The NYPD promoted Gormley awhile back to the rank of Lt. and there hasn't been any story line about his demotion to Sergeant. Thus it would appear that the writers, producers, actors, and others goofed in the title for Lt. Gormley by not correcting it during filming or post-production.
Goof: While driving the car and talking to Owen's mother, in mid-sentence Danny's cell phone switches from his left hand to his right hand between shots.

Continuity Goof: When Detectives Danny Regan, Kate Lansing, and Eddie Stone discuss why Otto is not the college supplier nor the killer, Detective Stone states that the drugs from the college campus are from Guadalajara and Otto gets his drugs from Durango. He also says dealers don't change their suppliers without risking serious repercussions, thus he knows Otto didn't do it. Another person is supplying drugs on the campus because the college campus drugs are coming from a cartel in Guadalajara.
Later in the episode after talking to the college drug dealer who names the real college supplier, Detectives Regan, Lansing, and Stone decide to go after him when he gets his next shipment. Detective Stone says they'll be coming from Durango. Yet, earlier he said they came from Guadalajara and dealers don't switch sources. This continuity switch between the two cities occurs throughout the remainder of the episode.
Goof: The Police Officer who approaches PC Reagan for smoking on the pier is wearing a gold braid on his cap. Only those in the rank of Sergeant or above wear a gold braid, Police Officers wear a plain black strap on their caps.
Goof: At the time Danny begins to question Ian Seroy, the only people who know there were three men involved are Sissy and the three men themselves. Sissy has not yet been found and none of the men have yet to even talk about the crime. When Ian breaks down and starts crying, Danny says to him, "What are you crying about? You crying about what you and your two cowardly friends did last night, huh?" At this point in the script, there is no way he could know there were three men involved.
Goof: When Raheem, in scrubs and mask, sneaks down the hospital corridor with a gun in his hand to kill Sammy, there is no ammo clip in the gun yet he fires a shot at the security guard.
The envelope for the condolence letter Commissioner Frank Reagan reads in his office is addressed to Linda Gates but her name is Laura Gates which is the name the Commissioner calls her.
As Danny and Jackie leave Jamie at the bar, Danny tells the bar man to "put it on my tab". This is in reference to a song called "Put It On My Tab", performed by Donnie Wahlberg's band, New Kids on the Block.
Jamie Reagan and Sgt. Renzulli are sent to 1345 West End Avenue for a call, but the door they knock on says 1301.
Goof: In the scene between Danny talking to Mary Jo in the car with her son, Jacob, there are several scenes between long and close up shots that don't match. In the long shots Jacob is sitting in his seat, while in the close-ups Mary Jo is holding him closely with her in the drivers seat.
The utility truck used in the robbery had Con Electric written on it. However, Consolidated Edison Company of New York (Con Edison), a regulated utility, provides electric service in New York City.
We learn that Erin is going through a divorce.
