Hugh has a miraculous recovery from open heart surgery during the previous episode.

Miller asks where Jeffrey Geiger is, and Watters tells him that he's on an extended leave of absence because of his daughter's illness. He's fine with that since he was planning to fire him. However, since Geiger had owned the controlling interest in the hospital, how was it sold without his involvement? And since he was chairman of the board by virtue of his ownership, once he had sold his share, he would no longer be chairman, and wouldn't need to be fired. And come to think of it, Geiger would have never sold the hospital to an HMO!

Francesca had a "torrid affair" with Hugh 11 years earlier in Boston.

Dr. Simon went to Yale University.

Logic flaw: Keith says he hasn't spoken to his aunt since he was 12. But she recognizes him in the elevator, knows his wife's and son's names, and knew he was a doctor. She admitted to him that she was purposely staying away from his family. And he knew that she was still working and still at Johns Hopkins. His mother is deceased, and they are both estranged from Keith's father. So how would they each know those things?

Keith's mother died when he was 12 years old from complications of diabetes. He hadn't seen his aunt since his mother's funeral.

This is the second time Dr. McNeil has had to improvise to perform emergency surgery in a non-medical setting. Previously, he performed surgery on a trans-atlantic flight with Dr. Austin.

Dr. Hanlon is called to the ER to help treat a serious trauma patient. However, as a plastic surgeon, her services wouldn't have been needed there.

Jeffrey Geiger's daughter Alicia is seven years old. However, in real time, she should only be four years old because she was born during season 1 in 1995. And earlier this season in the episode "Y'Gotta Have Heart", Jeffrey is trying to get her into an exclusive kindergarten. That would make her five.
