Katherine and Ray have their pasts come back as a temptation, which creates problems for Doogie in how to deal with his Mother's first love and Ray's old gang. Doogie and Vinnie work on a screenplay, where Doogie is a Civil War doctor. Meanwhile Doogie learns an important lesson at the hospital that the old pedestrian ways has advantages over a new Top Gun method.

The parents of a disabled child stymie Doogie, when they choose their religion and community beliefs over allowing him to walk through an operation. Meanwhile Janine comes up with a solution to her father's rejection of Vinnie; they're going to follow the example of Doogie's parents. She proposes they elope to Vinnie.
