I liked this ep but it seemed like Johnny Lee Miller was whispering for half the episode. It was throwing me off a bit, not because I had to listen more closely - I have no problem with that - but it seemed like he was acting all timid and unsure, which is out of character for Sherlock.

Answering to a comment after five months is a bit strange but I only recognised after one of the last episodes when everything I noticed before suddenly made sense. Sherlock is suffering from an Avoidant Personality Disorder which explains him acting so unsure and timid. That's his real personality. Most of the time he is able to mask his problems but in "Poison Pen" he was confronted with his past and so couldn't keep his "wall" up. When you read about causes and symptoms of AvPD everything fits. For example Sherlock never shakes hands with anybody. He only leaves the house when absolutely necessary and he even followed Joan Watson because he didn't trust her. The thing that struck me the most was, that he had a paniic attack when he unexpectedly met Gay in his living room and turned his back on her.moreless

Absolutely and you can even find more clues. Sherlock always positions himself in a room so that nobody can walk behind his back. He usually leans against a door, a wall or a piece of furniture. And what's interesting too is, that Joan Watson always talks to Sherlock before or when entering the room to make sure he notices her. When you are interested to know more here is a link:
