- Vondie Curtis-Hall (Henry Colton/Rena) was nominated for an Emmy Award in the Outstanding Guest Actor category for his work in this episode. He would return to ER in Season 8 as Roger Simmons, the husband of a woman with whom Benton has a child. Curtis-Hall would also go on to direct two Season 8 episodes: "Start All Over Again," in which he also guest starred; and "It's All in Your Head."

- Title: "ER Confidential" This episode's title is a reference to James Ellroy's 1990 crime fiction novel of the same title, which tells the story of three very different cops and how they uncover the corruption and scandal that intertwined Hollywood and the LAPD. In 1997 the novel was adapted into a film, directed by Curtis Hanson and starring Kevin Spacey, Russell Crowe, and Kim Basinger.

- The use of a sunflower to "take someone's temperature" is similar to a scene from Carry On Nurse, a 1958 British comedy film in which a very demanding patient has a daffodil used on him in the same way.
