Everything is falling apart, specially for Abby and Carter. Abby's brother ruins Carter's grandmother funerals and from that it is just falling apart. She wants to apologize and they both understand that they need to talk, but no - Carter always runs into emergencies, finds some patient to work with rather than talk and when Abby in the end wants to talk - he just yells and she leaves.

And Kovac. He manages to get the surgery for that sick boy and even thought he gets back, he will manage it. And Romano - oh, his life is one of them falling apart too. He puts his hand of fire and he realize - there is no getting better for him. It's over.
