Peter invites his family and friends for a party on board his fishing boat. During the party, Quagmire catches a fish which lands inside of Loretta's shirt. He asks her if he could have it back and she makes a snide invitation. While he reaches inside it to get his fish back, Cleveland serenely addresses Loretta and doesn't seem to think much of the fact Quagmire's hand is inside his wife's shirt. Quagmire makes a pick-up line and is confused and surprised to hear her take it seriously. In the middle of a game of charades, Joe accidentally falls overboard and Peter's two Portuguese employees rescue him from drowning using CPR.
Lois, feeling that Joe would have died if no one who knew CPR was there, insists that the family take CPR lessons. After succeeding in passing, Peter takes it too far when he sees a minor car accident and aggressively tries to save a conscious man's life. This action results in his CPR license getting suspended.
While Peter questions whether or not he should take the class again, Peter and Brian hear screaming coming from Cleveland's house. They bust the door open and feel awkward as they happen to see Loretta having sex with an unidentified white man (it turns out to be Quagmire).
Peter comes up with the notion that if Loretta if having sex with another person other than her husband, that means she is having an affair. They tell Lois, Joe, Bonnie and Quagmire about the incident and all they knew was the man in question has a tattoo on his left buttcheek. Peter volunteers to say something to Cleveland. At the Drunken Clam, Peter tells him exactly what happened and they don't know exactly who she was sleeping with.
When he arrives home, Cleveland mellowly asks her about her adulterous act and Loretta angrily informs him that she needs a real man. Cleveland thinks that it is somehow his fault she was driven to this, but Loretta thinks it is pathetic considering she is the one who cheated and he is apologizing. She proceeds to kick him out of the house.
Cleveland stays with the Griffin family and Peter thinks what he needs is a method of revenge. Thinking Quagmire would be knowledgeable about it, Peter and Brian walk over to his house while he is working out in briefs. Immediately, they recognize the tattoo on Quagmire's butt and they now know that Quagmire slept with Loretta. Despite Quagmire pleading with Peter not to tell Cleveland, he does anyway, but they are surprised to see Cleveland has little to no serious response. Lois tells him that Loretta did what she did because he's lost his passion and she needs a real man in her life. Peter tries to get a serious response out of Cleveland through several means, but to no avail. Finally, he begins to act as Quagmire with a face mask, but Cleveland only laughs at what he sees as an impression. Peter then grabs Brian and forces a Loretta face mask on him. As they both struggle on the floor, Cleveland's rage is clearly building. The now enraged Cleveland vows that for sleeping with his wife, he is going to murder Quagmire.
Realizing he went too far, Peter tries to protect Quagmire at Mayor West's mansion, but Mayor West's wackiness is too much. Quagmire returns home and tries to call Cleveland and apologize. Just then, Cleveland breaks the door open holding a baseball bat and vows that Quagmire will die for what he did. Once Cleveland finally has Quagmire cowering on the lawn, he is about to deliver a death blow. But upon seeing Lois, Chris, Meg and an indifferent Stewie looking on, he lets it go saying it's not in his nature to hurt anyone, regardless of what they had done. Peter tricks Loretta into seeing Cleveland again, but instead of reconciling, the bitter couple agrees to divorce.
Cleveland and Quagmire accept each others' apologies, and at Quagmire's insistence, they take their aggressions out in a boxing ring.
