Episode Summary

As Meredith puts Zola to bed and falls asleep, she begins to wonder -- what if her mother had never had Alzheimer's and she'd had loving, supportive parents? The reverberations of a happy Meredith Grey change the world of Seattle Grace as we know it. What if she had never met Derek in that bar and he had never separated from Addison? What if Callie and Owen had become a couple long before she met Arizona? And what if Bailey never evolved from the meek intern she once was?

  • Why Ellis Grey Sucks

    This episode wasn't actually all the "What if?" questions that Meredith asked in the opening monologue. For those that have been watching for long enough, and bothered to pay enough attention to remember some of the more interesting details, the scenario we saw was the scenario that Richard gave Ellis when Ellis was near death. He told her that he should have chosen her, and that Meredith would have had a caring father and a stable home, and Ellis would have been healthy and happy.



    Here's the problem with that "What if?" Ellis Grey, the surgeon, is a terrible, competitive person that beats down anybody and everybody. Ellis Grey, the mother, is just as neglectful and controlling as she ever was. This means that almost all of the changes are for the worse. I'll do this by character instead of timeline, and it'll only be what happened BEFORE the episode. Everybody else has already done reviews of the episode. I'm only going with characters we saw in the first episode of the series AND in this episode, though I'll also explain the write-offs (and in one case, on).



    - Chief Weber: With Ellis being chief instead of Richard, Richard isn't the respected elder surgeon that everybody knows and reveres. He's just another attending, who happens to be married to the Chief of Surgery. It means he has less authority than he normally would, and apparently has less confidence than he usually displays. However, he is a GREAT father, to the point where he adopted Meredith and she took his last name.



    - Meredith: Meredith is a shiny, happy person because she grew up with a loving father. She went to Mercy West for her residency instead of Seattle Grace, and became friends with April. She came to Seattle Grace after that hospital absorbed Mercy West, and presumably moved back in with her parents to be close to the hospital. Reid (you know, the first victim in the mass murder at the end of S6) took her place at Seattle Grace. She never knew Izzy, George, or Burke. She has also never met Lexi.



    - Derek: Since he didn't meet Meredith after coming to Seattle, he got back with Addison (okay, so I'm covering both of them). He's still miserable in his marriage, and with Ellis as his superior instead of Richard, he feels like nothing he can do will get noticed, and knows that Ellis prefers Addison, and Addison has a baby. Thus, he hasn't taught, and hasn't done any research, even that brain surgery method she developed with Meredith.



    - Bailey: Without Richard as the Chief pushing her to speak up and be better, she stayed the quiet and shy woman she was in that "Time Warp" episode. This meant that she never learned to advocate for her patients, even when she really needed to. She also isn't all that good when it comes to residents and interns. She motivates absolutely nobody, and just sticks back and lets Ellis Grey be Ellis Grey.



    - Cristina: Burke left because he slept with her, as Ellis Grey would have fired Burke, unlike Richard, who appreciated that Preston came forward, and that they were both unattached to anybody else. Since Burke left because of her, Ellis wouldn't give a damn what she thought of any other cardio attending, talented or not. Without Meredith as a friend, she never bonded with anybody else - well, maybe Reid, but Reid is still dead.



    - Alex: Karev never moved in with anybody else, and stayed a jackass. Without Meredith there, he blew his one shot with Izzy, meaning he never got close to her. His arrhythmia was misdiagnosed as vision problems, hence the glasses. He's still messed up because of family issues, but without an Alzheimer's trial, or a reason for Meredith to interfere in it, Karev's natural leadership skills came through, and he never betrayed anybody. One thing stayed the same, though - being around Meredith made him want to be better, and he screwed it up.



    - And Honorable Mentions: Ellis fired Preston Burke when he admitted to having a relationship with Yang. The reason O'Malley disappeared after failing his Intern exam was that Bailey wasn't a strong figure, and therefore didn't have the authority to encourage him to re-do his Intern year. Reid is still dead, but most likely because she ratted out Stevens, and therefore was killed when Izzy came back and shot up the hospital. Ellis isn't the type to care about plastics, so Sloane was never hired to the hospital. The rest is explained in the episode.



    In summary, Meredith may have wanted Richard to admit he should have chosen Ellis in the past, but truthfully, she would have been more miserable. Oh, and Yang and Meredith are destined to be friends, while MerDer was always destined to happen after Derek discovered Addison cheating on him.moreless

    I love Grey's Anatomy! I do, but I have to say that I was disapointed of this episode. I didn't really enjoyed watching this weeks episode, it was kinda boring. And ok, it is a alternative reality and what could have been .. Were the writers out of inspiration of storylines? I hope next episode will be better! But.. Grey's Anatomy still rocks!! :-) That's is why I don't give it a bad rating ;-)!!moreless

    ok this episode was awsome, alex as a geek awesome but hes still an ass , callie and owen married and having kids didnt like that much because it just didnt fit , christina wow awesome and mer very nice i loved that richard was her dad and lexie as a druggie i was a bit surpised but all in all great episode

    I'm not sure what all the fuss is about, this was one of the best and most classic Grey's Anatomy episodes ever! I mean that. Some will see this as merely a filler episode, but it was more than that...well it was filler, but the best kind of filler! A Filler that really resonated with the entire season and general happenings. This episode made me nostalgic of Grey's Anatomy when it first came about, and it did some very creative things with the characters. It was fantastic to see Kate Walsh as Addison again. Everyone was fantastic. It looked like they had fun with this episode and it was done pretty close to perfectly. While other bitch about it, I found the fun in it! It was beyond great.



    Would I watch this version of Grey's Anatomy week after week...maybe...it's interesting enough. If Shonda decided to do a AU spinoff...I would watch, especially if this episode is any indication of what that spinoff would be. Still, this was classic Grey's Anatomy.. Too great!!!!!moreless

      Alex: Here's the graft. I'm just suturing. (blood squirts on him) Whoa. Oh, my God. I might have nicked the cartoid.
      Ellis (looks up at the reporters in the gallery): You people are trying to kill me.

      Meredith: So, I started to do the repair--
      Alex: And she just stuck her hand in?

      Ellis: Get it together. Because if it comes down to a choice between you and her, you need to be very clear which one of you I will choose.

      Derek: I told you.
      Addison: I'm not asking for myself. I'm asking what I should tell the mother--

      Addison: So we'll talk about the preschool--
      Derek: It's gonna be late. I'm gonna stay and look into this clinical trial.

      Derek: Right. Everything's gonna be different when the baby comes.

      Derek (looking down): Of course I love you.
      Addison: Can you say it without looking at the floor? (Derek clears his throat, Addison shakes her head and walks out)

    • Meredith: Why aren't you doing a TEVAR?
      Cristina: I'm doing a direct approach. It's cooler.
      Meredith: A TEVAR is safer. It's far less invasive. You go in though the femoral, you don't have to crack the chest.
      Cristina: I already ordered the graft.
      Callie: You two might have worked out your surgical plan before this.
      Cristina: I did, and my surgical plan didn't include this nimrod.
      Meredith: No, it just included hacking up the patient like a psychopath.
      Callie: Girls, don't make me come over there and do it myself. Shut up and work.
      Cristina & Meredith: 10-Blade.

    • Ellis: So, June sounds good. Have you thought about where?
      Alex: Oh, M-Meredith and I thought about uh, Hawaii. That might be cool. Y'know, have everyone come out and--
      Ellis: Really, a destination wedding? (laughs) Alex, I'm running a hospital. I can't just dash off to-- You'll do it here. I'll book the Rainer Club. It's gorgeous.
      Alex: That sounds great.

    • Owen (after he punched a window): I'm sorry. I just got really...
      Cristina: Well, you won't need stitches... this time. I thought it was supposed to be getting better. Or are you gonna hit another intern?
      Owen: No. Why do you keep helping me? Why don't you just report me?
      Cristina: Because I'm a doctor.
      Owen: Please don't tell my wife.

    • Bailey: Uh, do me a favor.
      Alex: I'm going into surgery.
      Bailey: Yeah, just-- Tell Dr. Grey that the patient has a history of D.V.T and that the iliac veins may not be suitable for grafting.
      Alex: Whoa, whoa, whoa. What? Why didn't you say something?
      Bailey: J-Just... You're going in there. Just tell her.
      Alex: You tell her. She's your patient. I'm not gonna tell her she's wrong.
      Bailey: I-I mean, she-- she took the chart. Sh-She read it. Right?
      Alex: Yes, she knows what she's doing.
      Bailey: W- Well just tell her.

    • Addison: I need you to have lunch with the president of the board of the Bryson School tomorrow.
      Derek: Why are you talking about this now?
      Addison: Because, Derek, you're never home. You're alwasys spending the night in that stupid trailer on that stupid piece of land planning that stupid house that you're never gonna build.

    • (After Cristina catches Alex and April making out)
      Alex: Listen, if you tell Meredith or Ellis or anyone--
      Cristina: That surgery is mine. You can't just throw your girlfriend on it.
      Alex: I swear, Yang. I will fix it so you never see the inside of an O.R. again. I will bury you.
      Cristina: I'm not sharing my surgery. Change the board and get her off.

    • Jackson (initating Desi Arnez): Lucy... You got some 'splainin' to do.
      Lexie: I'm, uh, I'm just gonna go.
      Jackson: No, you're not. Because if you don't get this procedure you're going to die. Look, I get it. I know... it is scary. But I can tell you everything that's gonna happen.
      Lexie: Yeah, I already read the brochure.
      Jackson: Okay.
      Lexie: The automatic implanted cardioverter defibrillator is-- is placed beneath the skin below the collarbone. The electric wires th--
      Jackson: Wow.
      Lexie: Yeah, I have a photographic memory.
      Jackson: Okay. Well, you should go to med school.
      Lexie: Right. Or I could be a ballerina.
      Jackson: I'm serious. Okay? You should see all of this as a second chance. People turn things around all the time. You're smart. You're sharp. You're beautiful. You got a sister here you've never even met. You could have a whole new family. Let's start by fixing your heart, okay? (Lexie starts crying and Jackson hugs her)

    • Bailey: Dr. Grey is gonna graft an iliac vein to the portal vein, but that man's got bilateral deep vein thrombosis in his femorals and the iliacs won't be usuable. I mean, she's gonna look first, right?
      Callie (looking at Arizona): Who's that woman with Dr. Robbins?
      Bailey: I don't know. Some ortho surgeon.
      Callie: Oh. (laughs) I should've gone into ortho. I liked ortho.
      Bailey: Why didn't you?
      Callie: Well, when Ellis Grey says you're good for cardio...
      Meredith (walks up): Excuse me, Dr. Torres. I was wondering if I could do the aortic aneurysm today. This will be Dr. Yang's third, and I've only done one. So it's only fair--
      Callie: Webber, I know how many you've done. And I know what my residents need, okay?
      Meredith: My mother suggested that I ask you.
      Callie: Why don't you split it? You and Yang can, uh, do it together.
      Meredith: Thank you. (walks off)
      Bailey: Whatever Ellis Grey wants...
      Callie: You know it.
      Bailey: Yang's not gonna like it.
      Callie: Well, better than kicking her off the case. Bitch scares me.

    • (Watching Cristina practice stitches at lunch)
      Meredith: How many aortic aneurysms has she actually done?
      Alex: Two... In the hospital. She's probably done more at home on cats. (everyone laughs)
      Jackson: Does she eat with anyone, ever?
      April: No. That's what happens to you when you screw an attending. You're a pariah.
      Jackson: Unlike your approach of just never having sex with anybody?
      Meredith: No virgin jokes.
      April: Thank you.
      Jackson: Her and Preston Burke? Really?
      Meredith: And apparently it was so scary for him, he actually had to leave the state. (April laughs)

      Meredith: To get back on my bicycle.

    • Richard: Oh, hey, darling. What's up?
      Meredith: I have to steal a surgery from Cristina Yang.
      Richard: Well... Yang's always stealing surgeries. That girl gives me the creeps.
      Meredith: I know, it's just... Mom's operating today and I thought it would be nice to go in and see it, but she doesn't want me in there, so...
      Richard: Remember when you were learning how to ride a bike and you ran into the back of the Richmon's station wagon?
      Meredith: Vividly.
      Richard: What happened after that?
      Meredith: I sat on the curb and cried and bled all over you.
      Richard (laughs): Well, after that?
      Meredith: You made me get back on my bike.
      Richard: No. Your mother made you get back on, and you learned to ride a bike. She just wants you to succeed.
      Meredith: I know.
      Richard: I'm doing a proctoclectomy right now. You want in?
      Meredith: Single incision? (Richard nods) I probably shouldn't.
      Richard: Yeah, probably not.

    • Derek: What took you so long?
      Addison: I wasn't standing around, Derek. I had to make sure that the mother wouldn't bleed out.
      Derek: If we wait any longer she won't even be a mother. It's pressing against the trachea.
      Addison: I need to make sure the airway's secure.
      Derek: The bleeders can't wait. You have to work on the neck as I operate. It's gonna take us all day.
      Addison: I'm sorry. I know you weren't planning on spending the day with your wife.
      Derek: I'll see you in there.
      Addison: It's a beautiful day to save lives.
      Derek (shakes head): Nice.

    • Jackson: I'm gonna go out on a limb and assume you don't have any insurance. So, is there anybody I could call-- Parents, somebody?
      Lexie: No. Parents are dead.
      Jackson: I'm sorry to hear that. Is that from disease, family history?
      Lexie: My mother died of a stomach thing. My father killed himself.
      Jackson: I'm really sorry. (Lexie laughs) What-- What's funny?
      Lexie: I do have a sister. She's a half sister.
      Jackson: Okay. Nearby?
      Lexie (laughs): Very. She's a doctor... at this hospital. (laughs)
      Jackson: Are you kidding? ... Who? Who? Who is she?
      Lexie: Hey, we've never met. She doesn't know me. And trust me, she doesn't want to.

    • Cristina: Your heart stopped again. Because of the cocaine you've enjoyed, you're going to need a surgical implant-- An A.I.C.D pacemaker. If you have any questions about this procedure, here's an informative brochure. (hands Lexie the brochure and walks off)
      Lexie (to Jackson): She's a ray of sunshine.

    • Mrs. Taylor: Why can't you do the surgery?
      Bailey: Uh, the chief of surgery, Ellis Grey, will be doing the-- it. Uh, I mean, I can. I would. She just... There's protocols and she--
      Alex: Think of it as a huge upgrade. You just got handed the keys to a Jaguar and you've been driving like, a Civic. Good car, solid, dependable car. But Dr. Grey is... (Ellis walks in)
      Ellis: Hello, Alex. ... Mr. Taylor, Mrs. Taylor, I'm Dr. Grey. I'll be doing your pancreaticoduodenectomy today. I'll graft with a section of your internal iliac vein-- a new technique of my invention. By forgoing the use of the jugular, you'll have no visible scarring on the neck.
      Mrs. Taylor: Oh, well, that's nice--
      Ellis: I'll gladly have you sign these consent forms. It's a pleasure to meet you. I'll see you after we're done.
      Alex (whispers): Jaguar. (to Ellis as she's leaving) Doc-- Dr. Grey, any chance I could scrub in with you?
      Ellis: Alex, of course. Love to have you. (walks off)
      Alex (to Bailey): See? You have to speak up for yourself.

    • Mrs. Taylor: Why can't you do the surgery?

      Arizona: I think I do, too. (hugs Callie)

    • Jackson: Hi, there.
      Lexie: Oh, god. Ugh, what now?
      Jackson: Well, you O.D'ed, your heart stopped but you're stable. Can I get a name please?
      Lexie: Oh, so you can call the cops?
      Jackson: Nah, I just need anything really, so we don't mix up your pee with somebody else's.
      Lexie: You have my pee?
      Jackson: Yeah, you were kind of giving it away for a while there.
      Lexie (laughs): Spectacular.
      Jackson: Name?
      Lexie: Lucille. ... Ball.
      Jackson (laughs): You're a funny lady.
      Lexie: Well, that's how I make my living.
      Jackson: Alright, Ms. Ball. We're gonna get you worked up and send you on your way.
      Lexie: Hey, where am I?
      Jackson: Uh, Seattle Grace hospital.

    • Alex: Dr. Bailey, I am stoked for our whipple today. Are you stoked?
      Bailey: We, uh... No, we're not...
      Alex: You gotta speak up.
      Bailey: We're not doing a whipple today. I had it taken from me.
      Alex: Mandy. No.
      Bailey: Please don't call me Mandy.
      Alex: We talked about this. You can't let people walk all over you. You gotta stand up for yourself.
      Bailey: Not if it's Ellis Grey, I don't.
      Alex: Ellis is operating? Come on. The next best thing to operating is watching Ellis Grey do it. We gotta get in on that surgery, Mandy. Remember what we say?
      Bailey: I don't remember what we say.
      Alex: We create...
      Bailey: Oh, we create our own destiny.

    • Ellis (to reporters): Well, it's a stroke of luck, actually. I'm doing a surgery today and I'm planning to use the new grey technique. So, rather than a boring press interview, why don't you come on up to the gallery, take a few shots of the technique in action... and uh, we'll talk after? I'll see you in the O.R. (walks over to Mer)
      Meredith: That was brillant.
      Ellis: Well, why settle for another boring press release, right?
      Meredith: So, can I scrub in? I would love to see the procedure done firsthand.
      Ellis: Why? You're focus is cardio. You're not gonna get a cardio fellowship scrubbing in on your mother's whipple.
      Meredith: That's true.
      Ellis: Cristina Yang is doing a solo aortic aneurysm today.
      Meredith: I-- That came in last night and had I been here--
      Ellis: Yeah, but you weren't because you were too busy getting engaged. So, now that that's over, what do you want to do? Do you want to do an aortic aneurysm?
      Meredith: Yes.
      Ellis: Do you want me to ask Dr. Torres to give it to you?
      Meredith: No, mother. I can get it myself.
      Ellis: There you go.

    • Meredith: Good morning.
      Derek: Mmm. (hits the elevator button)
      Meredith: A clinical trial--
      Derek: This elevator's not working. (walks away)
      Meredith: What an ass.

    • Richard: All Ellis is saying is that she wants you to use your full potential. Like that clinical trial you used to talk about.
      Derek: I'm about to have a child, Richard. I don't have time for a clinical trial.
      Richard: I think that you should find it. I think that it'll do you good.

    • Addison (about Owen): Gosh, he's so good with the kids. How's he doing?
      Callie: He's better. Yeah. I think we're past it.
      Addison: Really?
      Callie: Yeah.
      Addison: That's great. I know... I've just heard that it can be... a hard road coming back. I mean, who knows what he went though over there.
      Callie: Yeah, well, he doesn't like to talk to me about it. But he does Skype with some of his old buddies that are still in Iraq, and, um, this guy Teddy seems to be helping him though it. And he hasn't had another episode since that one. So anyway, he's good. We're good... We're good now.
      Addison: I don't know how you do it. I mean, with the kids, too?
      Callie: Hmm, well... (laughs) You'll find out soon enough, right? (Addison laughs) Oh, besides, you and Derek will be fine. You two are perfect.

    • Meredith: I thought we had a rule about this in the hospital.
      Alex: Oh, I'm sorry. I just-- My god, you're so hot in that ring.
      Meredith: I do look good, don't I?
      Alex: Yeah. Hey, what did your parents think? I mean, what'd your mom say?
      Meredith: Oh, well, she loved it of course. And she adores you.
      Alex: Well, finally. It took a while.
      Meredith: Well, you were a jerk before you met me. She didn't know that there was a good guy in there but I did. (looks at the ring) Oh, I love it. And I love you.

    • Jackson: Good morning.
      Cristina: Are you talking to me?
      Meredith: Jackson, don't feed the animals.
      Cristina: Every single one of you can suck my--
      Alex (walks in): Hey! How's everybody doing this morning? Everybody ready to kick some butt? Let's run the board, guys. Avery?
      Jackson: Covering the pit.
      Alex: Noble work. Y-Yang.
      Cristina: Solo thoracic aortic aneurysm.
      Meredith: Well, I guess that means I'm doing Torres' post-ops.
      Alex: Kepner?
      April: Uh, I'm on Shepherd's service. Good Shepherd.
      Charles: Oh. Hey, I'm on bad Shepherd. Maybe we're gonna be the next Shepherds.
      April: No, we aren't.
      Alex: Alright, guys. The day's as good as you make it. Alright, let's get out there and keep Seattle Grace the country's best hospital. Come on! Yang, smile. Alright, let's move.

    • April (looking at Mer's engagement ring): Oh, my God. When?
      Meredith: Last night. I mean, I kind of thought he would and then, sure enough. Are you okay, April? 'Cause I know-- I mean, I knew this was gonna be hard for you but-- You are going to find someone, April. And Charles is really into you.
      April: Ugh. I know, I know. But Charles is--
      Charles (walks in): Hey, April. Charles is what?
      April: Nothing. (Mer makes a face) No, no, no. I am thrilled. Have you told anyone else?

    • Ellis: Meredith. Meredith.
      Meredith: Good morning, Mom.
      Ellis: Morning. Are you riding with me or with your father? Because if you're riding with me, we have to leave now.
      Meredith: Don't wait. I'll go with dad.
      Richard: Who's comin' with me? (to Mer) Oh, hi, darling.
      Ellis: Don't be late, either of you. It sets a bad example.

    • Ellis: I've been looking at Seattle Pres' numbers and I think that we can absorb them the way we did Mercy West. I want reports on where your departments can use extra residents and equipment.
      Addison: Congratulations on your third Harper Avery award. (they all clap)
      Arizona: Portal vein reconstruction. Pretty soon they're gonna rename the whipple the Grey.
      Ellis (laughs): Well, thank you. There's some press here today-- FYI. Okay, let's go. Trauma?
      Owen: Uh, adominal closure on an MVC, and we will wait to see what comes in.
      Ellis: Neuro?
      Derek: I have a laminectomy.
      Ellis: Just one? You got a golf game? (everyone laughs) You know what, people? I keep saying it. If you're not innovating, be generating. Cardio?
      Callie: Yang is doing a solo thoracic aortic aneurysm, oh, uh, and also I'll be checking in on my kid with the artifical lung who is waiting for a transplant.
      Arizona: Uh, yeah, can I just say for the record that he's my patient, and I don't agree with this method of treatment? The FDA has not approved an artifical lung on children.
      Callie: Well, I got emergency approval. It buys the kid more time. It keeps from being sedated and ventilated, I think it's worth exploring.
      Arizona: He should be sedated. Let his body heal. It's too risky.
      Ellis: It's innovative. Shepherd, this is what I'm talking about. Let's hope it works. Good work, Torres. General, Dr. Bailey?
      Bailey: Uh, yes, (quietly) an extended whipple.
      Ellis: Speak up.
      Bailey: I'm doing an extended whipple. Um, 47 year-old male, advanced pancreatic cancer with invasion on the portal vein.
      Ellis: Well, what are you using to graft the portal vein?
      Bailey: The... Um, the jugular.
      Ellis: I'll take that.
      Bailey: Excuse me?
      Ellis: I'll take your whipple. Send me the chart ASAP.
      Bailey: O-Okay, well, I...
      Ellis: Neonatal?
      Addison: A plyoric stenosis. Actually, two pylorics and...
      Richard (to Bailey): That's not a reflection on you, Bailey. She probably wants a surgery to take her mind off of all the Harper Avery fuss. Now don't take it the wrong way.
      Bailey: I'm not.

    • Meredith (opening voiceover): The baby you have is the baby you were destined to have. It was meant to be. That's what all the adoption people tell you. Anyway... I like to think it's true, but everything else in the world seems so completly random. What if one little thing I said or did could have made it all fall apart? What if I've choosen another life for myself? Or another person? We might have never found each other. What if I've been raised differently? What if my mother had never been sick? What if I've actually had a good father? What if...? What if...? What if...?

    • Meredith (closing voiceover): Your life is a gift. Accept it. No matter how screwed up or how painful it seems to be. Somethings are going to work out as if they were destined to happen... As if they were just meant to be.
