Meredith(voiceover:) To make it - really make it - as a surgeon - it takes major commitment. We have to be willing to pick up that scalpel and make a cut that may or may not do more damage than good. It's all about being committed, because if we're not? We have no business picking up that scalpel in the first place.

As the voiceover is heard, Burke and Cristina are performing surgery. The scrub nurse hands the stapler to Burke, he exercises his hand and waits for Cristina to pick it up.

After the OR, Cristina books a surgery for Burke for the next day at 10am, requests for his usual scrub team and reschedules another OR. A resident comes by and hands Cristina coffee. She gets the coffee and tells him that he's not scrubbing in. He complains that Burke always wants him in his surgeries. Another resident approaches her and asks if he could also scrub in. Cristina tells them that no resident scrubs in because it's too crowded and tells them to go away. She then reminds the nurse that the instrument tray be put on Burke's left side to give him more elbow room and that no gallery be allowed in OR2. The nurse comments that Burke has been very specific since coming back from work. Cristina threatens her that she will tell Burke she said that.

Meanwhile, George, Alex and Meredith watches as Cristina talks to the nurse. George says that Cristina has been running Burke's board since he came back. He asks them if Cristina thinks she's Bailey. Meredith answers that Cristina is just helping Burke. Alex says that she is taking advantage of the situation because she gets to scrub on every OR that Burke does. Mer tells them they are overreacting. Cristina is now arguing with the nurse and takes the pen from her and writes on the OR board. George is surprised by this. Mer is speechless and Alex says that maybe he should sleep with Burke.

At the locker room, Mer tells Cristina that she's been busy. She tells her that Derek hasn't called yet even though she told him that he broke up with Finn a week ago. Cristina doesn't seem to hear. Mer asks her if she wants to have a drink and catch up. Cristina refuses the offer and says she has to study for Burke's surgeries and then leaves. George then picks up his back and leaves. Alex looks at Meredith and she tells him that Cristina's busy. He stands up and leaves.

George follows Callie and tries to talk to her. He tells her that she should talk to him instead of ignoring him. Callie stops her tracks and says George then leaves.

Derek finishes his shower at his trailer and is surprised by what he saw. The woman, Nancy greets him. Derek asks her if she doesn't call first. A knock on the door is heard and as Derek turns around to see who it is, Meredith is already inside and before she can even finish her sentence, she notices Nancy who's on Derek's bed. Meredith is surprised and speechless then leaves. Derek tries to explain but she won't listen. Nancy sits herself on Derek's bed and tells him that she guesses that that was the slutty intern he cheated on Addison with. Derek defends Mer and tells Nancy that she isn't slutty. He then asks what she's doing in Seattle. She explains that Derek has bailed on Thanksgiving, on Christmas, that he lives in a trailer and is getting a divorce and then the slutty intern. Derek tells her that she doesn't really like her to which Nancy replies that Derek loves her.

At the elevator in SGH, George and Meredith are both silent. Izzie tells them that she's feeling good, really good. It's her first day back to SGH and she asks George and Mer if she looks okay and they both answer she's great. She tells them that she wants to get on the right foot and continues talking. The elevator door opens and Izzie tells them that it's her floor and gets out. Mer looks at George but doesn't speak. Finally George gets it and tells Mer that he'll be Cristina for her if she'll be Izzie for him. She agrees. George stops the elevator and they start talking. Mer tells him that Derek had a woman in his trailer who was ugly and very ugly except that she was tall and beautiful and that Derek was naked. George answers that McDreamy was doing the McNasty with a McHottie? That McBastard and asks if it was good enough. Mer nods in approval. Then it's George's turn. He tells him that Callie won't forgive him and won't talk to him and that she dumped him which he doesn't care about at all. Mer tells him that it's good because he deserves better and that he's George. She then asks if that was Izzie optimistic enough. He's stunned but agrees. Mer asks him what happened to them. He tells her that they are now the people that the people they want to be with avoid. She tells him that they have careers to think about and they don't need attention from them with perfect hair. He tells her that they should make a pact that there would be no more dating. Mer agrees. No more Derek and Callie but just work. They then knock fists in agreement.

Burke is meeting a patient and his wife. The wife tells him that he has heard so much about him. Cristina presents the case and mentions that the patient has a primary cardiac tumor. The wife then opens a file and tells them that her husband needs a cardiac autotransplantation. Burke is surprised by the procedure the wife mentioned. Cristina mentions that it is a Humpty-Dumpty surgery. He tells the wife that it is a complicated surgery and that there are lots of tests to be done. Cristina informs him that the tests were already done and that he is a perfect candidate. As Burke start to explain what the surgery is, Cristina butts in and tells them that they need to remove the heart, cut it open, scrape the tumor and put the heart together again. He tells them that it is risky but Cristina says it is possible. The wife tells them that she only wants what is best for her husband that is why they are in SGH. Cristina tells Burke that she has already scheduled him in the afternoon and that she wants to be as efficient as possible.

Chief Webber tells Bailey that Izzie has been cleared by the Legal Department. He tells her to explain to Izzie the parameters and that she understands them. Bailey is surprised that Izzie is being assigned to her. Chief says that she is her intern. Bailey tells him that she thought it would be better for Izzie to start fresh with another resident. He asks her if she doesn't want Izzie. Bailey replies that she wants her to succeed. Chief explains that there is still a fallout after the Duquette M&M and that he doesn't need to tell her that there are doctors in the hospital that have concerns about her judgment. He adds that Izzie was her mistake. Bailey asks if he has concerns about her judgment. Chief throws back the question at her and tells her that she's not gonna put those concerns to rest by avoiding taking responsibility for Izzie.

Addison meets with a patient who was transferred from Mercy West. George endorses the history of the patient, Noelle. She's 32 weeks pregnant with twins and it was noted that one baby is developing slowly than the other. Addison greets the patient. The guy introduces her as his fiancee. He tells them that Mercy West told them to transfer to SGH because the situation could get worse. Addison wonders why because Noelle is not in labor, is not bleeding and her vitals are okay. George then blurts out that Noelle has two uterus. He tells her that it is very rare.

Izzie meets with Bailey. She tells her that the paperwork took forever but is done with it already and that she is grateful that she is back. Bailey tells her that she should understand that there are rules to her probation and protocol to follow. Izzie says that she already knows because Legal already told her lots of stuff like the mandatory counseling, the extra lab hours and voluntary work with transplant patients. Bailey tells her that that was the stuff she was doing for the chief and that she was talking about what she'll do for Bailey. Izzie pulls out a notepad and asks what she's supposed to do. Bailey starts with the stuff that she don't get to do, she will not interact nor be alone with patients. She will be seen but not heard. She will not perform any procedures, the OR is off limits, no pre-op, post-op or anything that has to do with op. She has no authority and she doesn't have a choice in the matter. Izzie asks is there is anything she can do because she wants to be useful. Bailey tells her that she can't use her and that she's got to earn back the right for any of them to trust her again and until then, she will be shadowing a doctor each day. She then asks who she will be with today. Meredith is walking up behind her and then she asks why Bailey paged. Bailey tells her it's to Meredith. She reminds Mer that she has to make sure Izzie observes only. Mer is surprised.

While walking the hallway, Mer apologizes for what's happening. Izzie tells her it's not her fault and asks if she wants coffee or for Izzie to rub her feet. Mer tells her that she didn't ask for the assignment but Izzie tells her that she's fine with it. She then bumps into a man Derek is talking to. She apologizes and then greets Derek. Derek starts talking to Mer and tries to tell Izzie to go but she tells him that she can't leave her boss' side. Nancy comes in and tells Derek the trailer sucks but in the light of day, the land is nice and Seattle is pretty in the daylight plus he has a thing for ferries. Derek and Mer both say ferryboats. He introduces her to Mer and Izzie as his sister. Mer seems surprised and tells her that she is one of Derek's sisters. Nancy replies that Mer didn't think she was the wife since she already ran her off. Mer is stunned but smiles. Derek tells them that Nancy is visiting from Connecticut and that she is on her way straight back home. Mer tells her it was nice meeting her and leaves with Izzie. Izzie says McDreamy's sister is McBitchy. Derek confronts his sister.

Cristina and Burke argue about the Humpty Dumpty surgery. Cristina says she is in charge of the schedule. Burke tells her that she should have made sure that he didn't get a surgery like this one. She tells him that only a handful of people do Humptys and that he is the best in the West Coast. He tells her that the patient can go to Houston. Cristina asks him if he wants to go to the chief and that the patient will need the surgery or he'll die. Burke argues that she didn't tell him and she told the patient he'll have surgery but not him. She replies that she has been telling him all week. He asks if she's up for the Humpty. She replies that she has done her research and he should stop worrying. She mentions that the two of them are a well-oiled machine and walks away. Bailey approaches Burke and asks him about the rumors regarding her judgment since Denny's incident. He tells her that people just love scandals. She replies that he thinks it's just gossip and not actual concern as a doctor and he agrees.

Mer treats a patient with a burn on her hand. She tells Izzie to get materials for debridement and dressing. Patient explains that she should have known better and that whenever she is studying for the Bar that she can't focus on anything with all the stuff that she has to know. Izzie hands her the kit but she tells her to get the biosynthetic ones. Izzie tells her that they don't use those but Mer tells her that Sloan has changed the burn protocol a week ago. The patient tells her that before she takes some practice tests, she wanted some tea so she puts on a pot of water, sets the timer and after 30 minutes the smoke alarm goes off. Mer tells her that she's gonna be a lawyer. She tells her that all she has to do is pass the Bar. She mentions that she failed the last time but now she is ready and that she wants to pass.

Derek tells his sister that what she did to Mer was mean. She tells him that Kathleen called and told her to find out why the slutty intern's panties were hanging on the hospital bulletin board. Derek complains that he has four sisters and not one brother. And they wonder why he doesn't call anymore. Nancy tells him that he should feel free to explain about the panties anytime. He tells her that it's great to see her but he has a job and patients. She tells him that she has a patient at 35 weeks with twins and asks where her OB is. He tells her that her OB should be on a plane to Connecticut where she belongs. She doesn't listen and tells him that they've already covered the trailer but not the slutty intern and the divorce. Derek tells her that she's starting to sound more and more like their mother. She wants him to take it back and follows him in the elevator.

George is with Noelle and Greg and tells them that two uteruses is pretty cool and so is the engagement. She tells him that Greg talked her to it. Greg says that from the moment he saw Noelle, he was ham. George is surprised that he said ham but he explains that it's either ham or eggs. He's got to ask himself if he's the chicken or the pig. George is curious. He further explains that the chicken is involved in the meal but the pig is committed.

Mark asks Alex if he's free and he tells him he's absolutely free. He tells him to take his call with the DMV as he is on hold because there is a mix-up with the switching of his registration and license in Seattle. He leaves his phone with Alex. George walks up to Alex and Alex asks him how it feels to be the new gyne grunt. George tells him of his patient with two uteruses and is pregnant. Alex is surprised and George asks him if he's jealous. Alex says he's not and that he's busy and he's on hold for an important business with Sloan. As George leaves, Nancy approaches Alex and asks him if she just heard him say two uteruses. He asks who she is and before she could reply, Mark recognizes her. He calls her "Nancypants" and she calls Mark, "Loser". They embrace and he tells her that he wished Derek told him she was coming to visit. She tells him that Derek isn't really telling him anything these days. Mark says he's working on it. She asks what he's doing in Seattle and that if he is trying to torture Derek. He tells her that Derek's family and that he needed a change of pace plus he slept with his tennis partner's wife and he went out and bought a gun.

Addison walks over to them and asks if mom sent her out. Nancy tells her that she heard Derek has banned her from Seattle. Addison asks if Derek has told her that he was living in a trailer then they start laughing. Derek appears in the background and Nancy asks him to come over but walks away. Nancy follows him but turns to Addison and tells her he wants to see the two uteruses. Addie tells her to find her later. Mark tells Addie he misses Nancy and she agrees.

Back at Noelle's room, Greg is talking to the babies about football. Alex walks in and Noelle explains that when one of the twins get rowdy, Greg gets carried away talking to the babies and that he's excited that the baby is into sports already. He says he's daddy's little girl. Alex tells them that his voice calms the baby and that he's read articles on that recently. Addison and George comes in. Alex starts to leave and George stares at him. Addison says that she has good news for the couple. According to the tests that she's done, the babies are both healthy. The couple is relieved. Addison continues to explain that they are of different sizes because of a unique reason, they have different due dates. George tells them that the bigger one, the son was conceived a full six weeks before the daughter. Noelle and Greg are shocked. Greg tells them that he doesn't understand and that it can't be right and impossible. Addison claims that the tests are accurate in predicting due dates. He explains that they broke up when she got pregnant and he wanted to get married, she didn't, gave her an ultimatum and she left him. Noelle tells Greg that she is so sorry. He tells them that they were barely speaking six weeks after the conception let alone have sex. He tells them that he was not having sex. Noelle admits that she had a one-night stand. She didn't think that it would happen and that it was nothing. Greg couldn't believe she could do it and not tell him and left. George comments that Greg is not the father of both babies.

Addison walks over to Bailey and tells her that she accidentally broke the news of her patient's infidelity to her fiance. Bailey answers that no one is questioning her competence as a surgeon. Addison doesn't get it. The chief comes up to them and tells them that he needs a button and that he is down to his last clean shirt and he needs a button. The two ladies turn to him in amazement. He explains that Adelle always handles his buttons. Addison asks if he's called her. The chief says he doesn't want to bother her and asks both of them if they could help him. Addison excuses herself and says she has two uteruses to attend to. Bailey tells him she has many skills, surgical skills, if his button ruptures an esophagus, then she is her woman and then walks away. The chief can't do anything and just closes his coat.

Mark is treating Mer's burn patient and tells her that he heard that she burned her hand because she was studying. She explains that she's not even allowed to warm soup five days before taking the Bar so she burned her hand by burning her soup. He asks what happened when she grabbed the pot. She tells her that she dropped it and probably slammed it. Izzie is watching her carefully as Mer takes note of her story. Mark notices that some of the burns were pretty deep so she might have held on to the pot for a little while. She says she did then dropped it. Mark says that there are deep partial thickness burns and instructs Mer to prescribe some antibiotics. The patient says that it's bad and that she did it to her hand when she was supposed to take the bar in a few days. Mark tells her that even though the burn is bad, they can get her fixed up and she'll be fine and can take the test on Friday. She is shocked by what she hears and Izzie notices it.

Izzie tells Mer that their patient, Gretchen is lying and that something is off with her story. Mer tells her that she's supposed to be only observing. She tells Mer that she is observing and that something is off. She asks her if she saw Gretchen's reaction when Mark told her that she can take the exam on Friday. She comments that it was not relief but panic. Mer tells her it's okay to be nervous about being back at work and failing and that she hasn't been back a full day yet and she is already trying to bend the rules. Izzie replies that it is not about her but about their patient and that she is worried about her so she should get off her high horse. Mer reminds her that it is her patient. Izzie says fine, whatever and leaves.

Cristina is studying their patient's heart outside the OR when she hears Bailey mention the Humpty Dumpty surgery. She tells Cristina that Burke is doing an Humpty Dumpty and that she hasn't seen it done firsthand. She goes over the table to get a pen. Cristina asks what she's doing and tells her she's scrubbing in and writes her name on the board. She asks Cristina if she'll let Burke know. She says she will.

George tells Callie that he knows she is not talking to him anymore and that's okay but there is something he would like to explain and all she has to do is listen. He grabs Callie by the arm and turns her around to face him. He starts by saying that they are ham and eggs. He was the chicken and he wants her to know that. She put herself out there and she was committed but he was just putting the eggs on the plate. He isn't putting the ham because she was the pig. Callie now gives her attention to George. He continues that he was just involved but now he's committed. She asked him if he just called her a pig. George explains that it was a metaphor. He tells her that the point is she is not the pig anymore but he is. Callie stares at him then walks away. George watches her go and says to himself that he is the pig.

Izzie is walking down the hallway with her foodtray and see George, Cristina, Alex and Mer in one room. She tells them that she's been waiting for them for half an hour at the cafeteria and asks when did they get to eat in a patient's room. George introduces Izzie to the "really old guy" and tells her that they found him a couple of days ago and whispers that he sleeps all the time. Cristina tells her that it's quiet there and nobody bothers them. Izzie sits next to Cristina and Cristina asks her what she's doing. Izzie explains that she's doing nothing and she's literally not allowed to do anything. Cristina cuts her off and asks her what she's doing right now. She answers she is eating her lunch. Cristina tells her that if she want to socialize or talk about her day or get into a therapy session to do it at the other side. Izzie asks if she's serious and she says goodbye to her so she transfers to where George and Mer are. Alex asks about the girl with the two uteruses and asks if there are cervical changes in the ultrasound. George wonders why Alex cares but Alex says he doesn't. Izzie says she's that she's back. Mer gives her the look and she tells her she's not complaining about her, her boss. The really old man grunts but just changes position. Izzie says that what they're doing is weird. George tells Mer that she called Callie a pig. Alex asks if he said to her face while Izzie laughs. Mer asks what happened to the she's a corpse thing that they were talking about earlier. Izzie tells them how she can't get over how much everything has changed but nobody listens. Mer asks George how many times he called her and he answered just one. So Mer says she gets one too and wins the card game they were playing. She stands, picks up her coat and her coffee and leaves. Izzie asks what that is and George answers that they have a thing. Izzie wonders how they have a thing and she doesn't know about it and complains that they live in the same house and she sees him everyday. George tells her that it's a work thing. Cristina's pissed off and tells them that the whole point of hanging out with the really old guy is that he doesn't talk because he's sedated and that it is supposed to be a quiet place. She picks up her things and leaves. Izzie comments that Cristina's changed and everything else changed.

The wife of Burke's patient is complaining that she gave the other nurse her husband's list of medication and that he needed those pills hours ago. Cristina comes in and pacifies the wife and tells the nurse to go check the pharmacy again. She apologizes and tells her that she's not a yeller and she's become this horrible person who yells at people who were only trying to do their job. Cristina tells her that she is just looking out for her husband and she does what it takes to protect him and that doesn't make her horrible and it makes her smart.

Cristina goes back to the OR, checks the people inside and upon seeing that there aren't many people, goes to the OR board and erase Bailey's name in Burke's OR then leaves.

Noelle is leaning on her bed saying that Greg said he was leaving and he wasn't coming back. George tells Addison that Noelle is hyperventilating and the babies are having some sort of bradycardia. Addison tells her that she needs to get back on the bed. She is still asking if he really left. George tells her that she needs to take a deep breath. She tells them that she always gives Greg a hard time like how she hates weddings yet he likes them and how kids scare her to death yet Greg loves them. Addison asks if she's feeling contractions of any kind but she tells Addison that she can't do it without Greg. She suddenly screams after feeling some kind of pain. George and Addison helps her get on her bed. Addison tells her to stay on her left side and whatever she does, she shouldn't push.

Derek and Nancy are at Joe's eating lunch and Nancy asks Derek to tell her about the slutty girl. Derek gives her the look so she rephrases her sentence to slutty intern. He tells her that it was the slutty part that she had a problem with. Nancy mentions that Kathleen says she is not even single. Derek defends Mer by telling his sister that she is single, smart and she is a lot of things and that she is none of her business. Nancy is amazed because she says she's never seen him like this over a girl before, not even with Addison. He tells her he's never been like this over a girl especially Addison. She tells him that he shouldn't be bitter and that shame on Addison and all that. She says he's Mark and what should he expect. And asks him who hasn't gone there once or twice. She tells him that everybody sleeps with Mark like it's a rite of passage. She gets it and that Addison and Mark made a terrible mistake. Derek tells her that it wasn't a mistake and that it wasn't one night but months. He asks if she knew about it but she answers she has no idea. He tells her to shut up about it and just eat her lunch.

Cristina is still studying for Burke's OR and glances at the OR board. Mer comes over and sits beside her. She tells Cristina that Derek's sister is in town. Cristina doesn't answer. Mer asks her if she's mad with her because she's been trying to talk to her all week. Cristina cuts her off and tells her that not everything is about her. Mer asks what's the matter and tells her that George and Alex are saying --. Before she could finish her sentence, Cristina tells her that she doesn't care. Mer asks what her problem is. She answers that she just have bigger things in her life right now and walks away. Mer also leaves. Bailey comes in and notices that her name was erased on the OR board.

Burke prepares to start the Humpty Dumpty surgery. He stares at Cristina who stares back at him. She asks if he's ready then Burke requests for the ten blade signaling the start.

George leaves Noelle's room and gets some stuff over at the station. Greg sees him and asks what is happening. George tells him she thinks he left her. Greg explains that things are difficult to digest. His daughter, his little girl isn't even her little girl anymore. George says that Noelle is freaking out because he left. He asks him if Noelle is okay. George informs him that she's gone to premature labor and that Dr. Montgomery is about to do an emergency caesarean section on one of the babies to try to stop the labor on the other. Greg asks if they're delivering one of the babies right now and George tells him it's in a few minutes. He asks if it's the boy or the girl to which George replies ham or eggs then comes back to the room.

Alex is checking out a site on two uteruses and Mark catches him. He tells him that he wants a career in plastics yet he can't tear himself away from the baby catchers long enough to show him he wants it. Alex replies that being on hold with the DMV has nothing to do with his career in plastics. Mark tells him that it does because being on hold with the DMV has everything to do with him and he has everything to do with his career in plastics. Mark leaves. Alex is speechless, turns around, picks up Mark's phone and dials the DMV. A nurse informs nurse Tyler that Dr. Montgomery needs two units of B+ blood stat for her patient at L and D. Alex hangs up his call and proceeds to L and D.

Mer wraps Gretchen's hand in biosynthetic dressing and tells her she won't need skin grafts as Izzie looks on. She tells her that that is good news. Gretchen asks her if needing grafts would take long. She inquires how long would that take if she needs them. Mer answers that it will depend on the severity of the burn. Gretchen asks if it has to be more severe than her injury. Izzie clears her throat as Mer gets suspicious. She tells her that to be accurate, she asks her again about what happened. Gretchen tells her that they have been through this before and she wants to go home because she has a test on Friday. Izzie asks her if she burned her hand on purpose. Mer reminds her of what Bailey has said and Izzie says that it's okay and asks Gretchen if she burned her hand to get out of taking her test. She looks at Izzie and tells them that she can't fail the test again and explains why. Mer looks at Izzie like saying she's right. Gretchen tells them that it's what she's known for, Gretchen the failure. She asks them if they can imagine failing the Bar five times and says it's absurd and pathetic. She can't seat for two and a half days to prove again to everyone how pathetic she is.

Addison and George are now working on delivering Noelle's baby. She tells him that she doesn't want to alarm him or make him nervous cause he seems like a decent person but she has one hundred twenty seconds to get baby one out of uterus one while he is holding uterus two and if he makes a wrong move, he can tear up the uterine wall and kill the other baby. She asks him to do what he can to be careful. Nancy is amazed by what she sees. Alex looks on.

Meanwhile, Cristina and Burke work on their patient's heart. Cristina is placing the stay sutures. Burke tells her not to be afraid but Cristina finishes his sentence by telling him not to be afraid to grab a big piece. They then fish out a big piece of tumor. Burke inserts his finger to check the heart. She says that it was attached to the interventricular septum and mentions how it should be repaired. Burke is amazed that she studied her lessons well.

As Addison prepares to close Noelle's abdomen, George tells her that the baby is moving. She tells him that he needs to keep the baby still. George says he's trying his best. The monitor shows fetal distress. Nancy tells him that he's sending her to distress and that he needs to make the baby stop moving. George asks them how he'll do it. Alex comes closer and tells George to talk. Addison tells him to back away from the table but he doesn't listen. Alex tells George to talk to the baby but George doesn't know what he'll say. Alex starts talking about October 30, 1974, the fight known as Rumble in the Jungle, world heavyweight George Foreman is paired against Mohammad Ali, his first major fight after three and a half years out of the ring. George tells them that it's working. Alex continues talking and they proceed with the surgery.

The chief is at the OR working on his shirt when Derek comes in. He tells the chief that it's not gonna stay on and that he's giving it too much slack. The chief continues to sew on his button and tells Derek he's blocking his light. Derek adjusts the light and tells him to think of it as a basic corner stitch. He says that he can figure out how to sew his own buttons, thanks him and tells him he's a surgeon. Chief gives up and asks Derek to sew it for him and he'll get rid of Addison and Mark. He asks if he's really gonna do it but says no. Derek tells him it's fine and he's gonna do it anyway. Chief tells him that he hears he has a sister wandering the halls and asks if she's planning on moving to SGH too. Derek tells him that he hopes not. Chief tells Derek he knows it's been hard on him. Derek explains that he was like his brother. He has four very annoying sisters and Mark was his brother and it's hard. The chief says that divorce isn't all it's cracked up to be and he replies that he just wants it to be easy and to be over and move on. Chief tells him that he's in a surprising amount of pain. Derek then asks about him and Adele. Chief answers that he's sewing on a button for the first time in his life so what does that tell him? Derek tells him that technically he's sewing. Chief gives him a look then laughs, finishes and cuts the thread.

Alex gets out of the OR and throws his mask in the trash. As he goes out, he sees Mark waiting outside the scrub room. Mark tells him that they're clear and he knows that when he went inside that OR he was forfeiting his career in plastics. Alex tries to explain but Mark cuts him off and asks for his phone back. Alex hands him his phone and Mark leaves.

Burke's patient is wheeled out of the OR. He praises Cristina being well prepared. She tells him that she's always prepared. He tells her that she couldn't do it without him. She says thanks. Bailey approaches Burke and asks for a moment alone. Cristina leaves them but turns around and looks back at them. Bailey tells Burke that she didn't realize he was one of the doctors who doubts her and her abilities. He tells her that he doesn't. She tells him that her name was erased from the board and she assumes that it was him. He glances over Cristina's direction. Bailey asks him to tell her why he didn't want her in her surgery. He tells her he's afraid that he can't use her. She tells him she understands.

Derek is doing some paperworks when Nancy comes in and tells him he should have seen the two uteruses which was unbelievably cool and a cute baby to boot. He tells her he's glad she's enjoying her trip. She tells him to relax and that she leaves in two hours. He asks what she's going to report to their mother. Nancy replies that she'll tell her he's still Derek, still running in circles around all the women in your life. She cuts herself short and says that it's expected with four sisters and a dead dad. Derek says he's not running around in circles. Nancy asks him if he ever remembers the last time he was alone and that he has never been single ever. She says he's fine but not happy and he's not gonna be happy until he finds space, away from Addie, from the intern, just away and figure out what he wants. She kisses Derek goodbye and leaves. He thanks her for flying to Seattle and continues with his paper works.

Gretchen asks Mer where they are going and she tells her that they need to go upstairs. The elevator opens at the fifth floor. As they walk the hallway, Gretchen stops and asks where they are. Mer tells her they are on the psychiatric floor and that they need to place her on a 72-hour hold. Gretchen tells them she's not crazy and goes back to the elevator. Mer and Izzie follow her and she tells them that she needs to go home. Izzie tells her that if she goes home, she's gonna hurt herself again and that's her plan so she doesn't have to take the bar on Friday. Izzie tells her that she needs some help and that if the idea of taking an exam makes her hold her palm to a burner then she needs some help. She tells her that everyone needs help from time to time, someone to look out for them and make sure they're okay and make sure they are ready. Izzie says she has it and that Gretchen needs it right now. She reaches out her hand. Gretchen tells her she's not crazy and just didn't want to fail.

George and Addison check out Noelle's condition. She wakes up from anesthesia and asks them what happened and if the babies are okay. Addison tells her that the surgery went well and she had a healthy baby boy and the labor stopped on her baby girl. She asks if Greg has come back yet. Addison tells her he is not here. She then tells George to give her an update every half hour and to alert her of any fetal distress. She again asks what happened and Addison tells her she's okay. She asks again if Greg has come back. Greg answers he's here. He sits by her side and holds her hand. He tells her he saw their son and that he's amazing. She cries by what she hears. He then asks how their little girl is and touches her tummy.

Callie is doing some work when Mark approaches her and tells her it looks like she might need some cheering up. She tells him that it's not from him. Mark says that from what he recalls, he was good in cheering her up three times. She says it was not cheerful but dirty and she still says no then walks away. Alex overhears them.

Derek and Meredith meet at the stairs. Mer tells him that his sister doesn't really like her. He apologizes and tells her she's from the east coast. They stare at each other and Derek tells Mer that he should have called. He tells her that it's complicated and that he needs some time. Mer tells him to take some space. Derek agrees and she's fine with it.

There are times when even the best of us have trouble with commitment

Chief catches up with Izzie and tells her he has looked at Mer's evaluation of her on her first day. She's surprised and asks that she gets daily report cards now. Chief says that it doesn't have much to say and it was a difficult case nicely handled. She commends her for her work and welcomes her back.

Callie waits for George and tells him she was rude to him before and that she was sorry. She understands what he was trying to say that he was the pig and he was committed. The only thing is that they broke up. She has long been wanting to hear it from his to say it and yet he says it to her after they broke up. She is out of her element, breaks bones for a living, used to live in the basement, she wears last night's mascara to work in most days and she doesn't give a crap about what other people think of her because she is happily independent and a successful woman and she likes it that way. But when he says stuff like that, it just makes things too hard. She tells him not to chase her anymore unless he's ready to catch her.

And we may be surprised at the commitments we're willing to let slip out of our grasp. Commitments are complicated.

Cristina is in deep thought at the locker room. Mer approaches her and tells her she's here for her, whatever it is. Cristina doesn't answer. She tells her that whenever she's ready, she's just here. Cristina says thank you and Mer leaves.

We may surprise ourselves by the commitments we're willing to make.

True commitment, takes effort..

Cristina looks over at Burke

and sacrifice.

Cristina continues writing on the board and Bailey sees her. She takes a second look as Cristina erases the time of the OR and fixes Burke's schedule.

Which is why sometimes, we have to learn the hard way, to choose our commitments very carefully.
