Miranda gathers all the surgical residents and interns for a domino surgery which consists of 2 kidney transplants in 6 O.R.s. If they lose one donor, they could in turn lose them all.
There are complications with one young man whose father is paying him $10,000 to donate his kidney. Since the surgery is making national news, they all agree to turn a blind eye and let the surgery take place.
However, the surgeries are stopped when one of the donors refuses to donate her kidney after learning that her husband's donor has been his lover for three years. Miranda tells her that five peoples' lives are at stake and the donor finally agrees to do the surgery.
When a medical journal features Meredith and Derek's clinical trial, Derek ends up on the cover for "The Shephard Method." Meredith is upset that she did not receive any credit, and says that he didn't want people to know that he had help. He retorts that as a second-year resident, she does not quite deserve such credit. He tells her that she is being silly and Meredith tells him that "thank you" goes a long way." Derek gives Meredith a kidney in a jar and lets her know that he wouldn't have been able to do it without her.
Erica and Callie sleep together for the first time and Callie tells Mark the next day that it was not a good experience. She's confused when she finds out that Erica wants to see her again that night. Callie tells Erica she is not sure if she is into the whole touching thing.
She asks Mark to show her what to do because she wants to be good at it and he teaches her the Sloan Method. When she sees Erica, she orders her to take her pants off.
Izzie and Alex pick sides over who is to blame for the messed-up father and son relationship among the patients. Izzie alludes to Alex when she advises the father that some people are broken and that you can try not to care so much as not to get disappointed. Later, the father faces complications after the transplant and Alex who had agreed with the son earlier tells him to make peace with his father before time runs out.
The residents have to each give an intern to George and they argue over who to keep. The new residents don't respect George and Lexie is saddened that he did not ask for her.
After Cristina tells Joe that there are no real men left in the world, she finds out that Owen, the military doctor, is starting work the following day.
