In the beginning of the episode, before the dentist gets killed, he's giving the "dental gift packages" to the little kids. He gives out the first one, then to the left of the screen, by the inner room, you see a set of arms stick out indicating to the next kid to move up to the crew's spot so the kid will be in place to get his dental gift.
Even though Jim Kettle died of an asthma attack, his heart should have still been beating for a bit after he passed out from lack of air. Yet even though he crashed face first through a glass coffee table onto a hardwood floor and the M.E. says he had severe facial cuts, there is no pool of blood shown under him.
The coroner tells Goren and Eames that there was no evidence of sexual molestation, and they decided immediately that the dentist was an innocent victim. There are often no signs of molestation between pedophiles and the young boys they hurt (unless sodomy is involved). Unfortunately most pedophiles don't leave any physical evidence.
Dr. Elizabeth Rodgers: Jim Kettle was allergic to cigarettes, dust, mold, pet dander, car exhaust and cleaning fluids the guy was allergic to New York.

Store Manager: Kids? Most of my customers are kids. Robert Goren: Yeah, okay. (takes sales slips from manager) Any chance two of them came in when you opened the gate this morning? (hands slips to Eames) Alex Eames: Oh! Buying 20 pairs of $200 sneakers, various sizes? Store Manager: Oh, them. They were kinda scrawny. Alex Eames: Which one did you think was Dr. Norman Goldman?
Alex Eames: Sounds like a beautiful marriage. He was a workaholic, she took sleeping pills, they'd kiss in the morning.

This episode appears to be ripped from the headlines of the dental products found contaminated with diethylene glycol worldwide, beginning in 2007. Counterfeit Colgate toothpaste imported from China containing the contaminant was blamed for illnesses and deaths in Panama, Nigeria, America, and England. This followed the 2006 deaths deaths in Panamania from D.E.G.-tainted cough syrup, also from China.
