The Batlings all seem to vanish after Queen Bansheera initally falls into the open tomb.
After Carter says "The Omega Megazord's destroyed!", Ryan drops the Titanium Laser. In the next shot with them watching the Megazord explode, Ryan is seen dropping the Titanium Laser again!
In the end of the episode, all the Rangers, except Joel, are shown wearing their Lightspeed Jackets. With the exception of Carter, the only way the other Rangers could have gotten their Jackets was to go into the Aquabase, which was entirely filled with water at that point.
The Lifeforce Megazord is grossly oversized in this episode (it dwarves the Aquabase's towers, which comfortably held the Lightspeed Solarzord components).
Omega Megazord might have been the least damaged Megazord, but without Max Solarzord and the Trains, how could they hope to launch it's components? Wouldn't it make more sense to focus on those Zords first?
Carter, Chad and Joel's suits appeared to be severely damaged while the Rangers were in the Lifeforce Megazord, despite their suits not taking such damage prior to the Megazord's arrival.
Did Chad really have to call for the Omega Megazord, if Carter had already called for the Omega Zords?
How did the massive explosion caused by Diabolico and Olympius's fight not catch Lightspeed's attention?
Carter somehow knew that Queen Bansheera had destroyed Vypra, despite this information having no way of getting back to the Rangers.
When the Rangers are in the Lifeforce Megazord, the Sentai footage has them in a straight line inside the cockpit. however, the American footage has Dana and Kelsey behind Joel and Chad.
When the Lightspeed Megazord does its spin kick move, it is flipping the opposite way of every other time it has been used.
Bansheera claims that the Skull Cavern was destroyed, but it is seen in the next few episodes intact.

The "Previously on Power Rangers" scene includes a clip of Jinxer caring for Olympius in a cave, a scene not present in "Olympius Unbound" or any other episode.
When Aquafiend dries up the city's water supply, a woman's water cooler is also effected. Yet, water coolers are NEVER connected to water lines, but to tanks attached to the top.
Vypra is blasted directly by Chad's Mega Battle water cannon. She appars to melt away, as if the weakness to water all demons have led to her death. But no, she's fine in the next episode.
The Gatekeeper is seen in giant form destroying the city with Olympius, then after the commercial break, inexplicably vanishes, never to be seen nor mentioned again.
