When the bird drops the pen on Dr. Cox's head, you can see the pen bouncing off the head. In the next shot, Dr. Cox is holding the pen to yell at J.D. He can't possibly have caught the pen, as it clearly bounced away too far.
The one button is in fact missing on the Janitor's cell phone when it is in the bird's cage.
Sanchez the bird is a Chihuahuan Raven also called a White-necked Raven.
During the long "Turk as a father" dream sequence, it first says "10 years later" to show the pumpkin playing baseball. After that it says "21 years later" at the pumpkin's graduation. This means the Turks' pumpkin graduated college at 31 rather than 21. However, this could also mean 21 years after the initial incident, meaning the pumkin graduated at 21.
Janitor's line "Tienes mi corazon" when he lets Sanchez go is Spanish for "You have my heart" which is a common expression of love in Latin America.

At the end of the episode in the bar Turk reads off his phone "incoming call from Elliot Reid". However, when we see all of the numbers he has for Elliot earlier none of them say Elliot Reed, only "Elliot Home", "Elliot Work", "Elliot Cell" & "Elliot's Parents (in case Elliot isn't reachable).
When Dr. Cox is showing J.D. that his pen is a clickly top by clicking on it over and over, when he starts off doing it he is clicking it with his index finger, but when the shot changes to behind him, you can clearly see that he is clicking it with his thumb.
When J.D. and Carla are in Mrs. Wilk's room with the giant card, everytime the camera switches to Carla and J.D., there's nothing on Mrs. Wilk's tray. When the camera clearly shows all three of them together, you can see her napkin and food on her tray. At the very least, we should be able to see Mrs. Wilk's napkin when the shot changes to show only Carla and J.D., but nothing is visible.
Turk: Dude, you all right? You were gone for a really long time. J.D.: You're gonna be an awful father!
J.D.: Cabbage! Cabbage and Mark: Yes sir. J.D.: Oh no, Mark. I wasn't talking to you, I was talking to Cabbage. Mark: But my last name's Cabbage. J.D.: I know that, but I've nicknamed you "Wolfman" for your keen sense of smell. J.D.'s narration: And your ridiculously hairy torso. J.D.: Now, what can I do you for? J.D.: What did you think of the Transylvania dream sequence? Because I wrote it while I was on-call and the next morning and I read it and I was like, "What was I thinking?" Dr. Cox: Bethany, focus.
J.D.: I treat each and every one of you like unique individuals, ok? Take Gloria, for example. The woman's 400 years old. She needs to sleep a lot. I work around that schedule. Devin: "Make sure you clean out the gutters at the lake house so the front porch doesn't flood"? Turk: Ours was better than that.

Dr. Kelso: Patricia, it´s been a real pleasure having you here at Sacred Heart. And I certainly hope, the next time you fall ill, you remember us. Umm.. I´m.. not implying that you´ll get ill. But..it´s just that... you´re old and chances are good... Stop talking Robert!!
Dr. Cox: (Talking to Mrs. Wilk) I can't believe you´re finally getting out of this death trap. I mean, the odds were against you. You had a life-threatening disease... (looks at J.D.) a life-threatening doctor... (J.D. waves to Mrs. Wilk) Anyway, it was a pleasure treating you and... Mrs. Wilk: I know how hard it is for you to say something nice. So you can go! Dr. Cox: Thanks!
J.D.: Dr. Cox, may I borrow your pen? Dr. Cox: Not going to happen!! I had to strangle a nurse to get this clicky top. J.D.: Pleeaasse! I´m in the middle of a very threatening speech. Dr. Cox: Return this pen or die painfully!
Keith: How come I don't get a nickname? J.D.: The other's surnames lend themselves to a nickname. Keith: My last name is "Dudemeister"! J.D.: What can I do with that? I'm not a magician!
J.D.: You're late Keith. Which doesn't surprise me, because you're a terrible person. Keith: Sorry, I was helping with a gunshot victim downstairs. J.D.: Lies won't get you anywhere, Keith. Keith: I'm... covered in his blood. J.D.: That could be anyone's blood. Carla: Here's the gunshot victim from downstairs. Gunshot victim: Hey, my blood! J.D.: That's not your blood. Gunshot victim: Yes, it is! J.D.: Quiet time.
J.D.: What guy drunkenly kissed you when you were sleeping? J.D.'s narration: And it was at that moment that Turk and I remembered the incident we managed to block out for twelve years! Turk: Uh, you're okay with us not hanging around for a couple of weeks? J.D.: Totally!

Kareem Abdul-Jabbar also worked with Ken Jenkins (Dr. Kelso) in the mini-series The Stand (they only appeared together in "The Dreams" (the second part)).
The part were Dr. Cox tells J.D. about the clicky top pen it seems like a reference to the episode "My Self-Examination" but in that episode Dr. Cox tells Carla that he doesn't care about clicky tops or twisty bottom pens.
Credit sequence: During the episode we see J.D. fix lots of mistakes "Cabbage" makes. One of these is J.D. turning around an x-ray of a human chest. This is a joke/reference for the opening credit sequence, where J.D. puts the x-ray up and the chest is backwards. Many people thought of this as a mistake, but it actually was intentional. Bill Lawrence, the creator, said in an interview "the mistake" was intentional as he wanted the audience to think of the interns as inexperienced and therefore making a simple mistake. So when J.D. turns around "Cabbage"'s x-ray, it is ultimatly saying that J.D. has come a long way and is no longer inexperienced and making mistakes.
King Kong (2005): JD is giving Cabbage the Saturday off to go and watch King Kong, which was at the theaters arround the time this episode originally aired.
