When Keith makes the trucker horn gesture just after J.D. and the gang arrive at Kim's hospital, you can see the Janitor and Ted spooning on the couch if you look closely. Sam Lloyd stated that they were spooning in the episode commentary.
In the beginning of the episode, when J.D. goes to catch his alarm clock, he knocks Rowdy over. In the next shot, while J.D. is chasing his alarm clock, you can see in the background that Rowdy is still standing upright.
As The Janitor falls out of the RV while he is rolling he rolls towards the gate and just about hits it. But when the camera switches over, he is a couple of metres from the gate.
When Ted returns Dr. Kelso's RV, there is a shot of the two looking at it from the front. The passenger side mirror is still attached, and looking through the front windshield there is clearly no damage to the side of RV, yet it is present when Kelso walks around to the side.
When the Janitor is about to fall/jump out of the RV, though the front window there are clearly no cars visible. Yet when the camera changes to the side of the road there are many cars.
Just before The Janitor Falls out of the RV, you can see through the front window that the RV is on the left side of the road and that there is another lane on the right. But when The Janitor falls out he clearly falls on the dirt road on the right of the road and the RV is now in the right lane.
The doctor doing Kim's ultrasound at the end of the episode is played by TV Guide columnist Michael Ausiello.
When J.D. is talking to Keith about Elliot almost being sold, he says "there would be no sexual tension between us" meaning him and Elliot. This is either a mistake or a hint from the writers, because in Season 4, J.D. mentioned that all sexual tension between him and Elliot was gone. If this was not a mistake, J.D. might have been hinting that he and Elliot may still have some sort of feelings for each other.

Turk's ringtone was the melody from the song "Guy Love" featured in the "My Musical" episode. However, it would be interesting to know how Turk got a song that played only in someone's mind.
Turk: He can't possibly make this my fault...right?
J.D.: Dorian...aw, damn it, now I'm doing it.
J.D.: Hey, are you guys in?
Elliot: Totally! The last road-trip I took was with my sorority-sisters, so it will be nice to go with people who actually like me.
Keith: Why are you always so hard on yourself? Your sorority-sisters probably loved you.
(Pause)
Elliot: Keith, they tried to sell me.
J.D.: Y'know, Keith, if the buyer hadn't been an undercover Federali, Elliot would be a slave somewhere now...there wouldn't be any uncomfortable sexual tension between us, and you and I would probably be the best of friends.
Keith: (Grinning) Yeah?
(After the Janitor walked out of the moving RV)
J.D.: You guys, Turk told me how amazing it was when he saw Izzie's ultrasound. I can't miss this, I just...I won't.
Janitor: Can I say something? I don't know this young man, I wish I did. I admire his spirit and if he says we must continue onward, then I say I am in!
Dr. Kelso: (Referring to his RV) Check out that body. Bet you'd like to give her a ride, huh?
Ted: (Staring at an elderly lady) Ooh, you betcha.
Dr. Kelso: Over here perv.
Donna: You are ruining our neighborhood.
J.D.: Oh really, Donna, I'm ruining the neighborhood? At least I'm quiet. Were you guys having sex last night or raping a baboon?
Mr. Crosby: I wasn't home last night.
J.D.: My bad.
Donna: I am gonna beat your ass.
Turk: You could learn the sex, not tell Perry, hold it over his head, and play him like your little bitch-boy.
Jordan: Perry was so wrong about you! You're not an idiotic scalpel-toting, basketball-dribbling, blood-sugar-watching, idiotic man-boy.
Turk: (Flattered) Stop!
Turk: (To Dr. Cox) ...and you just gave my baby to Rochelle, the only nurse to get kicked out of the nursery for using a baby as a ventriloquist's dummy.
(Phone rings)
Turk: Mickhead! Gimme my phone!
Mickhead: It's mine.
Turk: Oh really? Does yours have a picture of Carla in the background?
Mickhead: Yes...
Turk: Ha! Trick question. Mine has Tyra Banks. See?
J.D.: Carla's got some serious moves! She can even do an inverted spread leg pole spin. (Elliot and Keith look at J.D.) If that's what they're called, it's not like I took a pole dancing class or anything.
J.D.'s narration: Nice cover!
Dr. Cox: Jordan...the boy already lip syncs into your tampons, must we put the final nail in his tiny gay coffin?
(J.D. believes that he can't go to Tacoma to go see Kim for her first ultrasound because he needs to find an apartment)
Dr. Cox: Doooorrrian...you have been wrong about so many things, that I'm not even going to say somethings wrong anymore. Instead, I'm going to say "it's Dorian". And the fact that you'd be willing to skip seeing that child's heartbeat for the first time is just...plain...Dorian. I mean, hell, Jordan's having her second ultrasound and I couldn't be more giddy. I can't wait to find out the sex of that unborn tax break.
Turk: Jordan doesn't let you know the sex until the baby's born.
Dr. Cox: Dorian again. I don't know if it's the hormones or if the baby's actually eating the bitch cells, but Jordan has softened. In fact, last night, she... (cringes) she asked me to cuddle.
J.D.'s thoughts: Is cuddling bad now?

In one of J.D.'s day-dream sequences Bob Kelso's camper talks to him like K.I.T.T. used to with Michael Knight (David Hasselhoff) in the series Knight Rider.
Hand of Hope
The scene where the Cox baby reaches out and grabs Turk's finger during the surgery appears to be a reference to a photo taken by Michael Clancy, originally featured in The Tennessean and USA Today in 1999, titled "Hand of Hope".
The photo showed a 21-week fetus' hand reaching out and holding onto a surgeon's finger during an in-utero surgery to repair spina bifida. The photo has circulated widely via e-mail since, and is frequently referenced in the abortion debate.
The scene is almost done exactly the same on "House" (Fetal Position, episode number 63, season three), when Dr. House performs exploratory surgery on a pregnant photographer and the baby's hand emerges and briefly grasps House's hand.
