At the end of the episode, a series of title cards inform the audience what happened to each member of the current Hammersley crew:

Dutchy is currently serving in Afghanistan.
RO continues to serve on patrol boats.
Swain's daughter is planning to join the Australian Navy.
Charge is now a mining engineer in Western Australia.
Mike was promoted to Captain and is now at NavCom.
Kate and Mike married. They are expecting their first child.
At the start of the episode, this message appears on screen: 'Mission Beach has been our home for all five series of Sea Patrol. We dedicate this show to all its brave residents and those of Dunk Island, Tully, Cardwell, and Innisfail – who faced the Cyclone Yasi – and won.'
In the last episode, 3-6, "Oh Danny Boy", Buffer gets an embarrassing tatoo on the back of his left shoulder. It was a heart with XO appearing on the banner. However, in this episode, 3-7, " Half Life" and the next episode, 3-8, "Red Sky Morning" (and perhaps others), Buffer appears shirtless, but there is no evidence of the tattoo.
When the bodies of the divers are transferred off Hammersley, the caption states that the other ship is HMAS Anzac (Hull number: FFH 150). The ship to which the bodies are transferred clearly bears the hull number L 50 which is in fact HMAS Tobruk.
Goof:
When the team board the small fishing vessel the water is dead calm with no waves. When the scene cuts back to the bridge of the Hammersley, you can see waves and white caps of the waves in the background.
Apparent Goof-
The mercenary's use of a hand held GPS is a critical plot point- but a hand held GPS would not be able to acquire satellites within the confines of a steel vessel and therefore would not be able to function. It is possible that there was some sort of a data link to an external antenna on the vessel topside, but another plot point had power cut off to the upper portion of the vessel.
Goof:
When the launch leaves the patrol boat there are four people on board, but as it's approaching the 'pirate' vessel in the next shot there are five people on it.
