Right after Duras slaps Worf, you can see his sash is not connected in the back. This allows Duras to more easily tear it off of him in the next scene, but it should have still been connected at the point he gets slapped.
Worf talks about his father being loyal to the Emperor. But in "The Rightful Heir" they stated the Klingon Empire hasn't had an emperor in over three hundred years.
There seems to be a problem with some of the sound effects in this episode. Specifically, in the scene where Worf first appears before the High Council, there is no sound effect for Worf getting punched, his sash being ripped off, or the sash falling to the ground.
For some reason, when Picard is having a private conversation with Worf, he accepts Worf's offer by responding in Klingon. Then for some reason he translates it into English - if he and Worf both speak Klingon, and no one else is there, who is he translating for?
Kurn: I never kill anyone at the supper table, Mr. La Forge.
Kurn: (preparing to dine) How long has this bird been dead? It appears to have been lying in the sun for quite some time.
Geordi: Well it's not dead, it's been replicated and you do understand that we cook most of our foods...
Kurn: Ah yes, I was told to prepare for that. I shall try some of your burned replicated bird meat.
Duras: Worf has made a choice and he will die for it. But you can still be safe. Let him stand alone.
Kurn: He is my brother. I will not betray him!
Duras: Then you will die for him!
Duras: For many turns the truth about Khitomer has been dormant... unknown. Now the truth has been revealed. The traitor, Mogh, sent the defense access codes to the Romulan patrol ships, allowing them to destroy the outpost. Thousands died on Khitomer. My father died on Khitomer. Their deaths must be avenged. Your father was a traitor. By posing this challenge, you are a traitor! You will not wear the emblems of our people. You are a fool, and your challenge can only result in a fool's death!
Worf: It is a good day to die, Duras, and the day is not yet over.
Duras: Then you must be ready to fight...something that Starfleet doesn't teach you.
Picard: You may test that assumption at your convenience.
This episode marks the beginning of a major plot thread for Worf, and would be revisited in the episodes, "Reunion," and "Redemption," (Parts 1 and 2.)
