Stan is staying with Dorothy while recovering from a major heart surgery. However, during the scene where he is wearing Dorothy's nightgown and eating cheesecake with the girls, his chest is exposed and there is no scar. Heart surgery like the one Stan underwent would leave a very big scar down the middle of his chest. In addition, someone who had heart surgery is supposed to be careful about what they eat, he certainly should not be indulging in cheesecake.
Blanche: I was only flirting. There's nothing wrong with that. It keeps you healthy, keeps you in shape, keeps your buttocks firm.
Dorothy: And I wasted all that time on jazzercise...
Rob: We're twins. We're here to see Blanche.
Sophia: She must be on the vitamins again.
Sophia: You call that wrestling, you pansy? If you can't take a crotch slam like a man then get out of the business!
Stan: Ask me anything.
Sophia: Allright, when the hell are you moving out?
Stan: Sophia, you kill me.
Sophia: I'd love to.
Stan: That's one of the interesting things about me.
Sophia: Please, you've been here two months. There's nothing interesting about you.

The title of the episode is a pun on The Man Who Came To Dinner, the 1939 play by Moss Hart and George S. Kaufman. It tells the story of a theater critic who, after hurting his hip, becomes the very annoying houseguest of an average family.
