When McCain, speaking of the Prime Minister, Harold Macmillan, says cynically that "poor old Harold's never had it so bad", he is making a play on words with regard to Macmillan's famous remark in an interview that most of the British people had "never had it so good." This phrase was used in electioneering by Macmillan's Conservative Party and would remain probably the most famous statement of Macmillan's entire political career.
