Whenever the Texan was looking at his dice and saw that he had rolled a seven (4 and 3) it only appears as seven in the close up shot. In every other shot it is an eight (5 and 3.)

This is not really a goof on the show itself but, in the closed caption, it misspells Campion as Campian.
Goof: Same footage has been used during the car crusher scenes.
When flight attendant Denise and David head to the cockpit to talk with Stephanie, David's shirt is tucked in. When they enter the cockpit, however, his shirt isn't tucked in.

In the shot of Carl flying through the air after Hulk threw him, he is wearing different clothes, has brown hair, and is holding a wooden board for no apperant reason. In the previous and next shot, none of these things apply.
It appears that during the transformation scene in the cab, a shot appears to be out of sequence. As the transformation is in progress, we see several shots of David's shirt ripping and his skin turning green. In the next shot of the cab driver telling David to "keep his shirt on," we can see David (though partially) in the background with his shirt still intact, his skin a normal color, but we hear the Hulk's gutteral growls. In the very next shot, we see the Hulk!
We see David take two shirts after changing back from the Hulk in a New York alley & it's obvious he's wearing both of them later when he unlocks the safe in Jason's office. So why doesn't he take off one & use it so he doesn't leave any fingerprints rather than using a pen to put the ledger in an envelope (& risking dropping it) & then wrapping the statue in brown paper that makes a lot of noise every time he touches it & making it more likely someone will hear him?

In one scene of the Hulk running down the streets of New York, you can see green slippers on Lou Ferrigno's feet.
The gorilla the Hulk fights with in the lab is obviously an actor in a monkey suit!!!
This episode marks the debut of the "Hulk Music" or "Creature Theme" that plays during almost every Hulk-Out. It is initially very orchestrated and fast but it will eventually become the normal "Hulk Music" that debuts in the episode "A Child in Need" (production order) or "Married" (airing order).

Near the climax, the Hulk pulls out a tree stump and throws it at the overhead helicopter, smashing its tail rotor and sending it out of control. However, in the very next shot, as the chopper spirals around, the tail rotor can clearly be seen intact.
When Jack McGee arrives at the hospital and introduces himself, Jack Colvin's lines are noticeably dubbed in post-production, as they were in the Pilot, to change the name of the newspaper he works for to the National Register. Later, when he goes to the Sheriff's office, again his dialogue is dubbed in post-production (though less noticeably). However, his ID that he shows to the Sheriff does have National Register written on it, in a shot most likely filmed (as many close-up shots are) in post-production.

In the close-up shots of the Hulk wrestling with the bear, you can see that the bear has green makeup smudged on its fur. Furthermore, prior to David "Hulking out" in the scene, although not explicitly shown, many of the close-up shots of David's face and the bear's arms, are quite clearly Bill Bixby wrestling with a man in a fur suit. And the "bear" that the Hulk throws through the air seems to be nothing more than a cheap child's toy, not even matching the build of the bear that the Hulk was wrestling with.
