The episode begins with Junior dreaming and worrying about his cancer. He's even dreaming of a way to turn Tony over to the Feds in exchange to be cured of cancer. Ironic to say the least, as Tony is sitting in the waiting room at the hospital worried about Junior while Junior is having this dream.

Paulie's taken to breaking Chris' balls more than usual. He's making him strip to prove he's not wearing a wire, refusing to pay money he loses to Chris in a pool game, and showing up at his house at 2:00 in the morning searching for money he claims Chris owes him and sniffing Adriana's underwear.

Tony becomes angry after hearing that Angie Bonpensiero (who is already receiving money from Tony to support her since Big Pussy 'left') was hinting to Carmela that her French Poodle needs an operation. Tony shows up at Angie's house smashing her brand new Cadillac's windshield, and warning her to go to him when she needs anything, not Carmela.

During a session between Carmela and Dr. Melfi (Tony skipped) Dr. Melfi realizes that Carmela is depressed. She recommends Dr. Sig Krakower. Carmela goes to see Dr. Krakower, who tells her to divorce Tony.

Junior's cancer also isn't going so great. Junior is taking chemotherapy at the advice of another doctor. Although he took the doctors advice, Junior still has all of his faith set on another man, Dr. Kennedy. Unfortunately, Dr. Kennedy takes Junior's decision to go with chemotherapy as a slap in the face, and doesn't return any of Junior's phone calls. However, after a few words to Dr. Kennedy from Tony and Furio at a golf course, Junior once again has the support and a home phone number for Dr. Kennedy.
