What's not to love about this episode? There's plenty of the lovably psycho Ralphie in it, for one thing. In fact, he is the impetus for the central plot: his joke about Johnny Sack's corpulent wife Ginny having a 90-pound mole removed from her ass sets off a byzantine series of events that nearly result in the whackings of both Ralphie and Johnny. Furio and Carm begin a flirtatious infatuation, and Meadow first hints at an interest in law instead of the medical career Tony and Carm prefer for her. Both of these critical plot points would see further development in later episodes.

While all of the performances are wonderful, Vincent Curatola (Johnny Sack)really gets his moment to shine in this episode, expertly segueing from totally believable tenderness and affection in his scenes with Ginny Sack to scarily intense anger in his scenes showing outrage over Ralphie's joke. Most notable for me was his deft comic timing in showing his exasperation at having to repeat the odious joke time after time while he was unsuccessfully seeking to sanction a hit on Ralphie.

In my opinion, a must-see episode: many of the events in this episode have far-reaching repercussions throughout subsequent episodes of the series.
