OK So in context it was just a joke but, still, it could've used a Soup-esque reaction.
Also on the subject of adults wearing diapers I would've loved to see Joel's reaction to Quagmire changing Joe's diaper in the Family Guy episode Airport'07 (that actually happened).

Not to mention he should've reacted to the Beverly Hills Chihuahua trailer (the Lou segment doesn't really count).
