The episode starts with Paulie receiving a tip from Vito about a Colombian drug money stash. When Paulie and Cary arrive, the place is not empty, as promised, and a battle ensues in which the 3 drug dealers die, and $1 million is recovered.
At the hospital Christopher and Bobby confront A.J., who has been trying to buy a gun to avenge his father's shooting. They strongly discourage him from that action.
In Tony's comatose dream, the threatened lawsuit for Kevin Finnerty arrives, and he goes to the monastery to try to clear things up. He also ponders whether the Alzheimer's diagnosis means maybe he really is Kevin Finnerty.
Meanwhile, Carmela has a chance encounter with Dr. Melfi, who offers her help. She is rebuffed, as Carmela feels she has "plenty of people around to talk to." Silvio needs advice of his own, as he tells Gabby that he's not entirely comfortable being the guy at the top of the pyramid. That doesn't change the fact that he's got responsibilites, namely deciding how to equitably split the Colombian score, a decision that neither Paulie nor Vito are thrilled with, as they see little value in kicking money up to Carm, given that if they wait Tony could die, leaving more for them.
Christopher seeks to re-enter the film business, via J.T. Nolan, who has fallen back into debt with Chris. The proposal is to help with a screenplay for a cheapie, "Saw meets Godfather 2," straight-to-DVD movie, and the debt will be erased.
Vito and Phil break bread, and Vito complains about the possible "money down the drain" kick-up to Carmela. Phil sympathizes, being the one who's, "keeping Ginny in butter brickle," but warns those are thoughts best dismissed.
Back in the dream, Tony finds a notice for a Finnerty family reunion, and decides to go and sort things out.
After seeing A.J. on TV talking about being a Soprano, Carmela flies off the handle, and finally seeks the counsel of Dr. Melfi. She fears the children's age and increasing wisdom now make them complicit in the family's life. She also reveals her feeling for Tony may once again be diminishing.
Dealing with the Colombian money split, and a beef between Bobby and Vito, cause Silvio to have an asthma attack that requires hospitalization, and creating a potentially dangerous power void.
Back at the hospital, a visit from Paulie sends Tony into distress, while in his dream he is ready to enter the Finnerty family reunion. Hospital workers race to stabilize Tony, and succeed as he finally awakens from his coma. Paulie and Vito scramble to get their overdue payments to Carmela, but she realizes the timing is not coincidence.
